Miami-Dade's diabetes rate and the two labels

CDC's 2023 numbers for Miami-Dade put diagnosed diabetes at 15.4% of adults against obesity at 29.5% [2], a high diabetes share for that obesity level. For those adults, Mounjaro's label covers blood sugar control in type 2 diabetes plus lowering major heart-event risk in high-risk patients [7]. Zepbound's label covers weight management and, in adults with obesity, moderate to severe obstructive sleep apnea [8]. Prescribing Mounjaro purely for weight is off-label. Compare them in Mounjaro vs Zepbound.

In SURMOUNT-4, after a 36-week lead-in, people switched to placebo regained an average of 14.0% of body weight over the next year, while those who stayed on tirzepatide lost another 5.5% [11]. Cheaper compounded offers in South Florida

FDA's March 2025 end to enforcement discretion for 503A tirzepatide compounding followed its finding that the shortage was over [12]. What remains legal is a one-patient prescription where the prescriber writes down a significant difference for that person [12]. These products are unapproved, and FDA reviews none of them for safety, effectiveness or quality [13]. UHealth, the University of Miami system, runs a bariatric surgery program; its page does not confirm medical weight management [[16]]. More: compounded tirzepatide.

From the Zepbound label

Do not use tirzepatide if you or a family member has had medullary thyroid carcinoma or MEN 2. Stop and call your prescriber for severe stomach pain that will not go away, which may be pancreatitis, or for swelling, rash or trouble breathing. Low blood sugar risk rises with insulin or a sulfonylurea. Stop Zepbound when pregnancy is recognized [14]. Call 911 in an emergency.
