Mounjaro vs Zepbound: same drug, different label

Mounjaro and Zepbound contain the same active ingredient, tirzepatide, made by Eli Lilly, in the same 2.5 to 15 mg strengths [1] [2]. The difference is the approved use: Mounjaro is for type 2 diabetes, while Zepbound is for weight management and obstructive sleep apnea [2] [1]. That label difference drives insurance coverage and price, with Mounjaro listed at $1,112.16 for 28 days [3] and Zepbound sold self-pay from $299 a month [4].

Quick answer

Yes, Mounjaro and Zepbound are the same drug. Both are tirzepatide injections from Eli Lilly, and both labels list single-dose pens or vials in 2.5, 5, 7.5, 10, 12.5 and 15 mg [1]. FDA approved Mounjaro for type 2 diabetes on May 13, 2022 [6] and Zepbound for chronic weight management on Nov 8, 2023 [7]. The brand you are prescribed depends mostly on your diagnosis, and that decides how insurance treats it.

Which is better for weight loss?

The molecule is identical, so the question is about the trials behind each label. Zepbound's weight trials reported an average loss of 15.0%, 19.5% and 20.9% at 5, 10 and 15 mg over 72 weeks in adults without diabetes, versus 3.1% on placebo [9]. In adults with type 2 diabetes, Zepbound's label reports 12.8% at 10 mg and 14.7% at 15 mg versus 3.2% [9]. Mounjaro's label is built around blood sugar, not weight [2].

Side effect rates also look different on paper. Nausea was 12% to 18% on Mounjaro in diabetes trials and 25% to 29% on Zepbound in weight trials [2] [9]. Those trials enrolled different people at different doses, so the gap does not mean one brand is gentler. See tirzepatide side effects and Mounjaro side effects.

Which is cheaper?

Without insurance, Zepbound is usually cheaper because Lilly sells it self-pay: $299 at 2.5 mg, $399 at 5 mg and $449 at 7.5 to 15 mg through LillyDirect if refilled within 45 days, checked Oct 17, 2026 [4]. Mounjaro's list price is $1,112.16 for four pens [3]. Lilly's Mounjaro savings card brings a covered prescription to as little as $25 (up to $150 off a month) or an uncovered one to as little as $499, but it requires commercial insurance and a prescription for an approved use [10]. We could not confirm a LillyDirect self-pay price for Mounjaro.

How compounded tirzepatide differs from both

Compounded tirzepatide is made by a pharmacy, not by Lilly, and it is not FDA-approved; FDA does not review it for safety, effectiveness or quality [13]. FDA has told telehealth companies that calling a compounded drug "generic," saying it has the "same active ingredient" as a brand, or implying it matches a brand's results is misleading [14]. Some compounded products also contain added ingredients: FDA's Sept 18, 2026 letter to Empower Pharmacy cited tirzepatide with niacinamide [15]. FDA had more than 730 adverse event reports for compounded tirzepatide as of May 31, 2026 [13]. Read compounded tirzepatide.

Switching between Mounjaro and Zepbound

People switch brands when a diagnosis or coverage changes. Because both are tirzepatide, Lilly says Zepbound should not be used with other tirzepatide-containing products [5], and Zepbound's label adds that it is not recommended with any GLP-1 receptor agonist [1]. The labels do not give a brand-to-brand switching schedule; the strengths are the same, and your prescriber sets the dose. The label schedule is on Mounjaro dosage. See switching GLP-1s.

Who each fits

  • Mounjaro fits a type 2 diabetes diagnosis, including the label's heart-risk use and children 10 and older [2]. Insurance and Medicare drug plans treat it as a diabetes drug.
  • Zepbound fits weight management or sleep apnea without a diabetes-only label, and is the brand Lilly sells self-pay at $299 to $449 [4].
