Tirzepatide prices at a glance

You can buy tirzepatide three ways: Lilly's brand drugs, compounded versions from telehealth programs, or through insurance. This page covers all three. For a single drug's detail see Zepbound cost or Mounjaro cost, and for semaglutide and the pills see the GLP-1 Price Tracker.

The costs behind the headline price

  • Plan length. Many low compounded prices assume a prepaid plan. Found's $239 rate is its 12-month plan; the 1-month plan is $359 [12]. IVIM's program is prepaid and non-refundable except as stated [2].
  • Membership and visit fees. IVIM adds $75 a month [2]; Henry Meds charges $30 per evaluation that results in a prescription [16]; Ro's membership runs $74 to $149 [11].
  • Dose step-ups. LillyDirect and FancyMeds charge more at higher doses [1] [14]. OrderlyMeds charges the same at every dose [13].
  • Late refills on Zepbound 10 mg and up: $699 instead of $449 [1].
  • Labs. Found excludes labs from its plan price [12].

FDA declared the tirzepatide shortage resolved on Dec 19, 2024. Its leeway for state-licensed pharmacies (503A) to compound copies ended March 5, 2025, and for outsourcing facilities (503B) on March 19, 2025 [17]. Today a 503A pharmacy may compound tirzepatide only for an individual prescription, and copies of an approved drug are barred unless the prescriber documents a change that makes a significant difference for that patient. FDA says it will not act against four or fewer such prescriptions a month [17].

FDA is tightening further. In February 2026 it said it intends to act against unapproved GLP-1 ingredients used in compounding [18]. In September 2026 it told a large compounding pharmacy that its tirzepatide mixed with niacinamide was essentially a copy of an approved drug, and that "significant difference" notes that were missing or repeated word for word did not support it [19]. Compounded tirzepatide is not FDA-approved, and FDA does not review it for safety, effectiveness or quality [33]. FDA had more than 730 adverse event reports for it by May 31, 2026 [4]. Full background: compounded tirzepatide and the FDA warning letter tracker.

Research-use-only tirzepatide

Vials sold as "for research use only" are not legal medicine. FDA has warned sellers of research-labeled tirzepatide and semaglutide sold to consumers, and says their quality is unknown and may be harmful [4]. We do not link to sellers. See research-use-only GLP-1s.

Dosing compounded vials

FDA has reports of people hospitalized after mismeasuring compounded GLP-1 doses [4]. The number of units in a dose depends on the concentration printed on your pharmacy label. Follow your prescriber's instructions and the pharmacy label, and ask the pharmacist if anything is unclear. Severe stomach pain that will not go away, signs of an allergic reaction or very low blood sugar need care; for an emergency, call 911.

Ways to pay less for tirzepatide

  1. Check your insurance first. If your commercial plan covers Zepbound, Lilly's card can bring it to $25 [8]. See does insurance cover Zepbound.
  2. On Medicare, see whether you meet the GLP-1 Bridge rules for the Zepbound KwikPen at $50 a month [9].
  3. Paying cash for brand: use LillyDirect and refill within 45 days [1].
  4. Comparing compounded programs: add up membership, visit fees and plan length, and check each program's FDA letter history. Our cheapest tirzepatide comparison does that math.

Why semaglutide is often cheaper

Brand semaglutide is priced flat: Wegovy pens are $349 a month at every standard dose [20], while Zepbound climbs to $449 [1]. Compounded semaglutide also lists lower, from $99 a month at Eden [21]. On results, the SURMOUNT-5 trial found average weight loss of 20.2% with tirzepatide against 13.7% with semaglutide at 72 weeks [22]. See semaglutide cost and Wegovy vs Zepbound.

Questions people ask

How much does tirzepatide cost per month?

Brand Zepbound costs $299 to $449 a month through LillyDirect when refilled within 45 days, and up to $699 for 10 to 15 mg if refilled later [1]. Compounded tirzepatide costs $117 to $399 a month in the online programs we track, checked Oct 17, 2026 [2] [3].

What is the cheapest way to get tirzepatide?

With commercial insurance that covers Zepbound, Lilly's savings card can bring it to $25 a month [8]. On Medicare, eligible people pay $50 a month for the Zepbound KwikPen [9]. Paying cash for the FDA-approved drug, LillyDirect's $299 to $449 is the lowest price we found [1].

Is there a generic tirzepatide?

No generic tirzepatide is sold. Compounded tirzepatide is not a generic, and FDA tells telehealth firms not to call compounded drugs generic or the same as an approved drug [29].

Is compounded tirzepatide legal?

Only in narrow cases. Since March 2025, a 503A pharmacy may compound it for an individual prescription when the prescriber documents a significant difference for that patient; outsourcing facilities may not compound it from bulk [17]. It is not FDA-approved [4].

Can you get tirzepatide in pill form?

The FDA-approved tirzepatide products, Zepbound and Mounjaro, come as injectable pens and vials [5]. The FDA-approved GLP-1 pills are Foundayo and the Wegovy pill, priced $149 to $299 a month [30] [20].

Can you buy tirzepatide over the counter?

No. LillyDirect fills Zepbound only with a prescription [1]. Products sold without a prescription as "research use only" are not legal medicine, and FDA warns their quality is unknown [4].

How much is tirzepatide on Ro without insurance?

Ro sells brand Zepbound KwikPens at $299 for 2.5 mg, $399 for 5 mg and $449 for 7.5 to 15 mg, plus a membership of $39 the first month and $74 to $149 a month after (checked Oct 17, 2026) [11].

Will tirzepatide get cheaper?

Lilly has cut LillyDirect prices before: on Dec 1, 2025 it lowered Zepbound vials to $299, $399 and $449 from $349, $499 and $499 [31]. We have not seen a further cut announced as of Oct 17, 2026.
