If you have type 2 diabetes

About 11.9% of adults in the city and 12.6% in Los Angeles County have diagnosed diabetes [3] [4]. For them the relevant semaglutide is usually Ozempic, which is approved to control blood sugar in adults with type 2 diabetes and to lower cardiovascular and kidney risks [6]. Medi-Cal's 2026 change targeted adult weight loss; non-weight indications can still be covered if medically necessary [2]. Commercially insured patients whose plan covers it may pay as little as $25 a month with Novo's savings offer, capped at $100 a month in savings [7].

At $349 a month, a year of the pen is $4,188, or about 5.1% of the city's $81,939 median household income [5]. The pill is taken each morning on an empty stomach with up to 4 ounces of plain water, then a 30-minute wait before food, drink or other pills [8]. Our pill vs injection page compares the two.

The January 2027 list price cut

On January 1, 2027, Novo Nordisk plans to lower the list price of Wegovy, Ozempic and Rybelsus to $675 a month [9]. List price is what insurers start from, not what most cash buyers pay, so the NovoCare self-pay prices in the table may matter more to you than the list price change.

Compounded products and California checks

Online sellers in California still market compounded semaglutide. None of it is FDA-approved or FDA-reviewed for safety, effectiveness or quality, and FDA says the sodium and acetate salt forms are separate active ingredients it knows no lawful basis to compound [10]. Pharmacies can be looked up with the California State Board of Pharmacy [11], and physicians with the Medical Board of California [12]. Our compounded semaglutide page has more.

Warnings to read first

Acute pancreatitis is on the label, so stomach pain that is severe and keeps going needs urgent care [8]. The rodent thyroid C-cell tumor boxed warning rules out anyone whose own or family history includes MTC or MEN 2. Pairing it with insulin or a sulfonylurea makes lows likelier. Face or throat swelling signals a serious allergic reaction. Per the Wegovy label, stop once pregnant and at least 2 months before trying to conceive. Emergencies: 911.
