"Bioidentical" is a marketing term clinics use for estradiol, progesterone and testosterone products, covering both FDA-approved prescriptions and compounded mixes that are not FDA-approved.
"Bioidentical" on a Beverly Hills menu
The word says nothing about quality. Clinics apply it to FDA-approved estradiol and to custom compounds alike. Compounded bioidentical hormone therapy is not FDA-approved, and FDA does not review it for safety, effectiveness or quality: a National Academies review found insufficient evidence for its clinical usefulness and said it should be limited to patients allergic to an ingredient in an approved product or who need a dose form that is not available [4]. FDA commissioned that review because it was concerned patients may not know the uncertain benefits and risks [5]. The Menopause Society lists concerns about overdosing, underdosing, impurities and sterility [6].

Checking a prescriber in California
The Medical Board of California licenses physicians and offers a public license lookup [3]. Look up the doctor who will actually prescribe, not only the clinic's medical director, and note any public discipline.
Age, timing and the 2026 label changes
FDA's November 2025 action removed boxed-warning language on cardiovascular disease, breast cancer and probable dementia from menopausal hormone labels, kept the endometrial cancer warning on systemic estrogen-alone products, and recommends starting systemic HRT within 10 years of menopause or before 60 [7]. FDA approved the first revised labels for six products on February 12, 2026 [8]. In a city where one in four residents is 65 or older [1], a first-time systemic prescription after 60 should come with a careful talk about your own risks.

Testosterone for men
FDA's 2025 class-wide update removed the boxed cardiovascular warning from testosterone labels, kept the limitation on age-related low testosterone, and added a blood pressure warning [12]. The AUA requires total testosterone under 300 ng/dL on two tests with symptoms [13]. See TRT and online TRT clinics.
