Online TRT is testosterone therapy prescribed after a telehealth visit and lab work, with medication shipped to the patient or sent to a pharmacy.
How online TRT works
- Lab work first. Programs use a partner lab, an at-home kit, or labs you bring. TRT Nation accepts outside results for total testosterone, CBC, CMP, estradiol, and PSA if you are over 40 [1].
- A telehealth visit with a licensed clinician, who decides whether you qualify. The AUA standard is two tests below 300 ng/dL plus symptoms or signs [7].
- Medication ships to you or goes to a pharmacy. Fountain TRT ships a daily cream monthly [2].
- Follow-up labs and visits, every 3 to 6 months at Fountain [2] and every 90 days on Hone's Premium plan [5].
What to check before you sign up
- Who prescribes. Get a name and check the license with your state board [8].
- What you get. FDA-approved testosterone, or a compounded product such as enclomiphene or a cream.
- Controlled-substance rules. Testosterone is a Schedule III controlled substance under the Controlled Substances Act, as the AndroGel label states [15]. Ask how the program handles prescriptions and refills in your state.
- Where you live. TRT Nation does not serve AL, AK, AR, MO, ND or HI, and requires age 24 or older [1].
- The company's record. Search our FDA warning letter tracker.
- Price and commitment. Monthly fee, labs, medication, and whether you must prepay. Defy says it has no subscriptions or contracts [9]; Marek says you can cancel anytime [10].

Cheapest online TRT
TRT Nation's $99 a month was the lowest program fee we found, but labs cost extra unless you bring your own [1]. Hone's $25 Basic plan is lower on paper, yet medication is billed on top [5]. If you already have a prescriber, generic testosterone cypionate costs about $20 to $31 per vial with a coupon [12]. See TRT cost per month for the full breakdown.
Providers we do not recommend
Hims (testosterone). In July 2026 the FTC, Utah and Los Angeles County sued Hims & Hers, alleging undisclosed subscription enrollment, hard cancellation with continued charges, and sharing health data with Meta and Snap for ads [3]. FDA sent Hims a warning letter on September 9, 2025, over compounded semaglutide claims [13]. Its testosterone line is compounded enclomiphene, not testosterone. The lowest price, $99 a month, requires a 10-month plan paid up front, and injectable TRT and Kyzatrex are listed as "coming in 2026" [4]. We do not link to it. Read our Hims testosterone review.
We have not reviewed every brand. Before you sign up with any other program, check the prescriber's license, the pharmacy and the FDA letter database. Compare in-person options on find a TRT clinic, and see enclomiphene if a program offers it instead of testosterone.
Safety note
The 2025 class-wide label change added a blood pressure warning to testosterone products that lacked one, after studies confirmed that testosterone raises blood pressure [14]. Testosterone gels carry a boxed warning because children who touched application sites have developed signs of male puberty [15]. In TRAVERSE, the testosterone group had more pulmonary embolism (0.9% vs 0.5%), atrial fibrillation and acute kidney injury [16]. Call 911 for chest pain, sudden shortness of breath or other emergencies.
