Coupon prices vary by pharmacy and ZIP code. A fill of 90 tablets or 4 patches lasts as long as your prescribed schedule allows, so ask the pharmacist how many days your fill covers.

Hidden costs that change the total

  • Visits. Many programs charge per visit. Midi Health lists $250 for a first visit and $150 for a return visit [2]. Ask how often follow-ups are expected.
  • Memberships. Evernow starts at $35 a month [4]. Hone Health charges $25 or $155 a month on top of medication [8]. A membership keeps billing whether or not you refill.
  • A second prescription. Care plans may include progesterone as well as estrogen, and each has its own price. Midi and Evernow both list progesterone among what they prescribe [10][4].
  • Labs. Some programs order blood work. Hone lists a $65 biomarker test and consult [8]. Ask whether any lab is required and who bills it.
  • Hidden prices. Alloy shows prices only after an intake form [11]. Get the full number in writing before you pay.
  • Brand-only forms. Brand gels and vaginal rings averaged hundreds of dollars per bottle or ring in 2024 [6].

Ways to pay less for HRT

  • Ask whether an FDA-approved generic (estradiol tablets or patches) fits your plan. Those were the lowest prices we found [1].
  • Compare coupon prices at a few pharmacies. For estradiol vaginal cream, the coupon price was $36.74 against a $310.37 retail average [3].
  • Check TrumpRx. In May 2026 it expanded to more than 600 generic drugs and added discounts from Amazon Pharmacy, Cost Plus Drugs and GoodRx [12]. We did not confirm whether estradiol is on its list.
  • Use HSA or FSA funds. Midi Health and Alloy both say they accept them [2][11].
  • A generic version of Premarin (conjugated estrogens) tablets was approved, FDA said in November 2025 [13].
  • Skip pellets and compounded creams if an approved product works for you. Pellets alone can run $1,200 to $2,000 a year [9].

Does insurance cover HRT?

It depends on the plan. Original Medicare (Parts A and B) may cover visits but generally not the hormones; Medicare Part D can cover them, with the cost set by the drug's formulary tier [14].

Some telehealth clinics bill insurance for the visit. Midi Health is in network with most PPO plans, treats Medicare as out of network (self-pay only) and does not accept Medicaid [2]. Evernow accepts some plans, including BCBS, Anthem, UHC and Aetna, for video visits [4]. Gennev lists Aetna, Anthem and Cigna [15]. Call your plan with the drug name and strength to get your copay.

How HRT compares with other options on cost

Approved generics are the low end. Compounded "bioidentical" products and pellets cost more and are not FDA-approved; see bioidentical hormone costs and hormone pellet cost.
