Medical weight loss is weight treatment run by a licensed prescriber that may use FDA-approved drugs, lab checks and changes to food and activity.
Cook County numbers behind the demand
About 2,731,585 people lived in Chicago on July 1, 2025 [5]. Across Cook County, CDC PLACES estimates 31.0% of adults had obesity in 2023 and 11.8% had a diabetes diagnosis [4]. Those are the groups weight drugs are labeled for: Zepbound is approved for adults with obesity, or with overweight plus a weight-related condition [6].
Coverage is the sticking point. Lilly, quoting an IFEBP survey, says roughly half of Americans with job-based insurance have no benefit for obesity drugs [7]. New to all this? Start with the weight loss guide.

Three routes to a Chicago prescription
- Your own primary care doctor or an endocrinologist. Often cheapest when your plan pays for visits, even if the drug is still self-pay.
- A storefront weight clinic. Fees are rarely posted. Ask whether the monthly charge buys visits, labs, the drug, or only the prescription.
- Telehealth. A prescriber with an Illinois license sees you by video or form and sends the order to a pharmacy. Compare programs on online GLP-1 programs.
Northwestern Medicine, Rush University System for Health and UChicago Medicine are the city's big academic systems. None of them posts a weight management price list that we found, so call primary care or endocrinology and ask. Questions to bring are in our clinic finder checklist. Drug-specific local pages: tirzepatide in Chicago and semaglutide in Chicago.
The drug's price, before a clinic adds anything
Wegovy tablets start at $149 for 1.5 mg, then $199 at 4 mg and $299 at 9 or 25 mg; Wegovy pens are $349 for any dose from 0.25 to 2.4 mg [3]. Zepbound is $299 to $449 at LillyDirect, more if a refill runs late [2], against a $1,087 list price [9]. Foundayo, Lilly's tablet, is $149 to $299 [8]. A Chicago clinic quoting one flat monthly figure is either folding its fee into one of these prices or selling a compounded drug. Ask which, and read compounded tirzepatide if it is the second. Illinois coverage: Medicaid, Medicare and job-based plans
KFF's January 2026 list of 13 states paying for obesity GLP-1s through Medicaid leaves Illinois out [1]. Members with type 2 diabetes are a separate case, since Ozempic and Mounjaro are diabetes drugs. For Chicagoans on Part D, the temporary Medicare GLP-1 Bridge runs July 1, 2026 to December 31, 2027 at a $50 monthly copay for those who qualify [10] [11]; see our Medicare GLP-1 page. A worker whose plan already covers Zepbound may pay as little as $25 with Lilly's savings card [12].
Checking a clinic before you pay
- Search the prescriber in the Illinois Department of Financial and Professional Regulation license lookup [13].
- Know the state rule: the Illinois Medical Practice Act bars licensees from sharing fees with others in return for referrals, apart from listed arrangements [18]. Referral bonuses are a warning sign.
- Look the company up in our FDA warning letter tracker. In a single wave announced March 3, 2026, FDA warned 30 telehealth firms [14].
- Ask for the pharmacy's name and whether the product is brand or compounded.
- Get refund and cancellation terms in writing before the first charge.

When to stop and call
Because tirzepatide and semaglutide caused thyroid C-cell tumors in rodents, Zepbound and Wegovy carry a boxed warning, and no one with medullary thyroid carcinoma or MEN 2 in their personal or family history should use them [15] [16]. Severe stomach pain that persists may be pancreatitis; stop and get care. Lows are likelier alongside insulin or a sulfonylurea [15]. Wegovy should stop at least 2 months before a planned pregnancy [16]. Swelling of the face or throat, or trouble breathing: call 911.
