VO2 max test: cost and where to get one

A lab VO2 max test cost $315 at the Hospital for Special Surgery in New York on Oct 17 2026, or $470 as a full metabolic profile with lactate testing [1]. The reason people buy it: in a study of 122,007 adults, the fittest group had an 80% lower adjusted risk of death than the least fit (hazard ratio 0.20), though the study was observational [2].

A VO2 max test measures the highest rate at which your body can use oxygen during hard exercise, a standard measure of cardiorespiratory fitness.

Why fitness gets so much attention

Mandsager and colleagues followed 122,007 adults who had treadmill tests for a median of 8.4 years. Compared with the lowest performers, the highest performers had an adjusted hazard ratio for death of 0.20 (95% CI 0.16 to 0.24). For comparison, the study reported hazard ratios of 1.41 for smoking, 1.40 for diabetes and 1.29 for coronary artery disease, and found no upper limit to the benefit of fitness [2].

Observational, not proof

The study shows that fit people lived longer. It does not prove that raising your number will add the same years, since fitter people may differ in other ways [2].

Lab test vs other options

A lab test measures oxygen use directly while you exercise. Watches and apps give estimates. The Mandsager study measured fitness from treadmill performance, not from wearable estimates [2]. If you are tracking change over time, using the same lab and protocol each time keeps results comparable.

Where to get a VO2 max test near you

  • Hospital sports medicine and performance programs, such as HSS in New York [1].
  • Some longevity programs include fitness testing; compare in the longevity clinic guide.
  • If you are weighing an executive physical, ask whether it includes a treadmill or VO2 test.

Hidden costs and safety

  • Add-ons such as lactate testing raise the price; HSS charges $470 for VO2 max plus lactate versus $315 alone [1].
  • A retest to track progress is a second full fee.
  • People with heart symptoms, such as chest pain or fainting with exercise, should see a doctor before a maximal test. A preventive cardiologist can advise.

Insurance and HSA

HSS lists its metabolic tests as cash prices [1]. A fitness test ordered for a sports or wellness goal is usually self-pay. A cardiopulmonary exercise test ordered by a doctor for symptoms is a different service and may be billed to insurance. Ask your HSA administrator whether a wellness VO2 test qualifies.

Pair fitness with body composition on a DEXA scan, and heart plaque with a coronary calcium score. Epigenetic clocks are covered in biological age tests.

Questions people ask

How much does a VO2 max test cost?

The Hospital for Special Surgery in New York listed $315 for a VO2 max test, $470 for VO2 max plus lactate, and $210 for resting metabolic rate on Oct 17 2026 [1].

Why does VO2 max matter for longevity?

In an observational study of 122,007 adults, the fittest had an adjusted hazard ratio for death of 0.20 compared with the least fit over a median 8.4 years, and no upper limit of benefit was seen [2].

Is low fitness worse than smoking?

In the Mandsager study, low fitness compared with elite fitness carried a larger hazard than smoking (HR 1.41), diabetes (1.40) or coronary artery disease (1.29) did. The study was observational [2].

Does insurance cover a VO2 max test?

HSS lists its VO2 max test as a $315 cash price [1]. Wellness fitness tests are usually self-pay; ask your insurer about a doctor-ordered exercise test for symptoms.

Is a watch VO2 max estimate the same as a lab test?

No. A watch estimates; a lab measures oxygen use during exercise. The Mandsager data came from treadmill testing, not wearables [2]. HSS charged $315 for a lab test [1].

How often should I retest VO2 max?

There is no guideline interval for wellness testing in our research. Each retest at HSS costs the full $315 [1]. Ask the lab or your doctor what interval makes sense for your goal.
