Enclomiphene vs TRT compares an unapproved, compounded drug that clinics market for low testosterone with FDA-approved testosterone given as injections, gels or oral products.
Quick answer
Testosterone has FDA labels, a large heart-safety trial and cheap generics. Enclomiphene has none of those: it is compounded, and we could not confirm trial results to cite. Clinics market it as an alternative to testosterone. If a prescriber suggests it, ask why an unapproved drug fits you better than an approved one.
Why this page is marked incomplete
Our research could not confirm published trial results, side-effect rates or an FDA statement for enclomiphene, including FDA's position on whether pharmacies may compound it. We show what is known and say plainly where the comparison has gaps.
Side by side
| Testosterone (TRT) | Enclomiphene | |
|---|---|---|
| FDA status | FDA-approved [1] | Not FDA-approved; compounded [2] |
| Approved use | Low testosterone with primary or hypogonadotropic hypogonadism; not age-related low T [1] | None |
| Forms | Injection, gel, oral (Jatenzo, Kyzatrex, Tlando). Clinics also sell creams and pellets; ask whether yours is an FDA-approved product or compounded | Compounded product from telehealth and men's clinics |
| Dose | Set by each product's FDA label and your prescriber | No FDA label, so no approved dose |
| Key safety data | TRAVERSE: major heart events 7.0% vs 7.3%; more pulmonary embolism, atrial fibrillation and kidney injury [4] | Not confirmed in our research |
| Label warnings | Blood pressure warning class-wide (2025) [5]; gels boxed for secondary exposure [1] | No FDA label |
| Price, Oct 17, 2026 | Generic cypionate about $20 to $31 per 1 mL vial with coupon [3]; TRT Nation program $99 a month [6] | Hims from $99 a month on a 10-month prepaid plan [2] |
| HSA/FSA | Depends on program | Not eligible at Hims [2] |
The differences that matter
- Review. Approved testosterone products went through FDA review and carry labels that FDA updated in 2025 [5]. Compounded enclomiphene skips FDA review for safety, effectiveness and quality.
- Evidence. TRAVERSE randomized 5,204 men to testosterone gel or placebo [4]. We found no comparable enclomiphene trial in our research.
- Diagnosis. Both should start with lab-confirmed low testosterone: the AUA uses two tests below 300 ng/dL plus symptoms or signs [7].
- Cost structure. Generic testosterone is cheap per vial; enclomiphene's lowest published price ties you to a 10-month prepay [2].
Who each may fit
We cannot tell you which will work for you. Testosterone is the option with FDA approval for men whose low testosterone has a medical cause [1]. Enclomiphene is offered by programs such as TRT Nation, Maximus and Gameday [6][8][9]; ask the prescriber what evidence supports it for your situation and which pharmacy compounds it.
Switching between them
Enclomiphene has no FDA label, so there are no labeled switching instructions. Any change should come from your prescriber, with labs before and after. If you move to testosterone, the label now warns that it can raise blood pressure [5], so plan for blood pressure checks.

Safety note
The 2025 class-wide label change added a blood pressure warning to testosterone products that lacked one, after studies confirmed that testosterone raises blood pressure [5]. Testosterone gels carry a boxed warning because children who touched application sites have developed signs of male puberty [1]. In TRAVERSE, the testosterone group had more pulmonary embolism (0.9% vs 0.5%), atrial fibrillation and acute kidney injury [4]. Call 911 for chest pain, sudden shortness of breath or other emergencies.
