Concierge medicine is a practice model in which a doctor charges a membership fee for extra access and services, while usually still billing insurance for covered care.
Is concierge care priced for Orlando incomes?
Orlando's median household income was $72,336 in 2020 to 2024 Census data [5]. A $2,400 retainer, close to the MDVIP fees we found [7], would take about 3.3% of that figure before any insurance premiums. The AAFP notes that concierge practices typically serve higher-income patients, while DPC serves patients across income levels [4]. That is why many Orlando families compare the two models side by side in concierge vs DPC.

Florida's DPC law and what it means for you
Florida passed a direct primary care law in 2018 (SB 80, signed March 23, 2018) [6]. DPC practices charge a periodic fee and do not bill insurance fee-for-service [4]. Since January 1, 2026, federal law lets people with a high-deductible plan keep contributing to an HSA while enrolled in DPC, and pay DPC fees from the HSA, if fees are no more than $150 a month for one person or $300 for a family [8] [9]. Concierge practices that bill insurance on top of a retainer do not qualify for that rule [8]. Start with direct primary care or find a DPC practice.
Medicare and concierge fees
About 11.3% of Orlando residents are 65 or older [5]. Medicare does not cover concierge membership fees [10]. A doctor who accepts Medicare assignment cannot charge the fee for services Medicare already covers, and must give you an Advance Beneficiary Notice if the fee includes items Medicare will not pay for [10]. Our concierge medicine and Medicare page lists questions to ask.
Before you sign a membership contract
- What exactly does the fee buy: an annual physical, same-day visits, phone access?
- Does the practice still bill your insurance for visits and labs?
- How long is the contract, and is there a prorated refund if you leave?
- Can you pay quarterly? MDVIP, for example, allows quarterly, semi-annual or annual payment [11].
- Can the fee be reimbursed from an FSA or HSA? Ask in writing and check with a tax adviser.
