Concierge care in a lower-income city

Philadelphia's median household income, $61,953 [4], is about half of Seattle's [7]. A $2,500 retainer would be about 4% of the city's median income before any insurance premium. That is why direct primary care, with typical fees of $50 to $100 a month per the AAFP [8], matters for many readers here. The AAFP notes that concierge practices typically serve higher-income patients [8].

Pennsylvania rules

DPC Frontier lists no direct primary care law in Pennsylvania [5]. Federal HSA rules still apply: since January 1, 2026, DPC fees up to $150 a month for one person can be paid from an HSA without losing eligibility, while practices that also bill insurance are excluded [11]. Check any physician with the Pennsylvania State Board of Medicine [12].

For Medicare patients

About 14.7% of Philadelphians are 65 or older [4]. Medicare does not cover concierge fees, and the fee cannot include extra charges for Medicare-covered services; noncovered items in the fee require an Advance Beneficiary Notice [13]. See concierge medicine and Medicare.

Longevity testing in Philadelphia

Prenuvo lists a Philadelphia location [6], with a Whole Body scan at $2,499 and a Focused scan at $1,199 [14]. The American College of Radiology does not recommend total-body MRI screening for people without symptoms or risk factors [15]. A coronary calcium scan usually costs $100 to $150 cash [16]. Compare full-body MRI costs and coronary calcium score.
