How we check

Concierge and longevity medicine in San Antonio

Concierge medicine in San Antonio means paying a yearly membership on top of insurance, and the nearest Texas fee we could document, $2,400 a year at an Arlington MDVIP practice, would equal about 3.7% of the city's $65,056 median household income [1][2]. Direct primary care, legal in Texas since 2015, often costs less [3].

DPC in Texas: the law and the HSA rule

Texas law has defined direct primary care outside insurance regulation since HB 1945 in 2015 [3]. Federal rules changed in 2026. Under IRS Notice 2026-5, a direct primary care arrangement does not block HSA contributions if it meets these conditions [5]:

  • Fees of no more than $150 a month for one person, or $300 if it covers more than one person.
  • Care only from primary care practitioners, paid by a fixed periodic fee.
  • No general-anesthesia procedures, no prescription drugs other than vaccines, and no lab work beyond what an ambulatory primary care office usually does.
  • No separate billing to insurance for services inside the arrangement.

Go over the cap and you can still be reimbursed, but you lose the ability to contribute to an HSA while enrolled. Employer-paid DPC fees cannot be reimbursed from the HSA [5]. These rules took effect January 1, 2026 [6]. See the DPC guide and how to find a DPC practice.

  3. What is included: annual physical, labs, same-day visits, after-hours access?
  4. What are the cancellation terms?
  5. Is the doctor in good standing with the Texas Medical Board? [11]
