Ozempic dosage is the once-weekly semaglutide dose schedule for type 2 diabetes set out in Section 2 of the FDA label, from a 0.25 mg starting dose to a 2 mg maximum.
The Ozempic dose schedule on the label
| Step | Dose | How long | Notes |
|---|---|---|---|
| Start | 0.25 mg | 4 weeks | Starting dose [1] |
| Step 2 | 0.5 mg | At least 4 weeks | First maintenance option [1] |
| Step 3 | 1 mg | At least 4 weeks | If more blood sugar control is needed [1] |
| Step 4 | 2 mg | Ongoing | Maximum dose [1] |
Ozempic is taken once a week, on the same day each week, at any time of day, with or without meals [1]. The lowest dose is 0.25 mg. The highest is 2 mg. Searches for the "highest dose of Ozempic for weight loss" do not have a label answer, because Ozempic is not approved for weight loss [1]. Wegovy, the semaglutide brand labeled for weight management, uses a different schedule: 0.25, 0.5, 1 and 1.7 mg over 16 weeks, then 1.7 or 2.4 mg, with 7.2 mg allowed for some adults [2]. See Wegovy dosage for that schedule.
Which pen gives which dose
| Device | Total in device | Concentration | Dose per injection |
|---|---|---|---|
| Single-patient-use pen | 2 mg in 3 mL | 0.68 mg/mL | 0.25 mg or 0.5 mg |
| Single-patient-use pen | 4 mg in 3 mL | 1.34 mg/mL | 1 mg |
| Single-patient-use pen | 8 mg in 3 mL | 2.68 mg/mL | 2 mg |
| Single-dose prefilled syringe | 0.25 mg, 0.5 mg or 1 mg in 0.5 mL | Varies | One dose |
The 4 mg pen holds four weekly 1 mg doses and the 8 mg pen holds four weekly 2 mg doses, based on the total and per-dose amounts in the label [1]. The 2 mg pen is listed as delivering 0.25 mg or 0.5 mg per injection [1]. We could not confirm from the label text available to us today how many doses that pen provides at each setting, so check the carton or ask your pharmacist. The label lists the 4 mg pen as delivering 1 mg per injection; it does not list it as a 0.5 mg pen [1].
Clicks, half doses and splitting a dose
The Ozempic label does not describe counting pen clicks to measure a dose. It lists only the doses each pen is designed to deliver [1]. Questions such as "how many clicks for 0.25 on a 2 mg pen" are about dialing a dose the device is not labeled to give. That is off-label dosing, and we do not publish click counts. If your prescriber wants a dose your pen does not deliver, ask the pharmacist for the pen made for that dose.
The label gives one weekly dose, not two half doses, and does not describe splitting doses across the week [1]. For compounded semaglutide in vials, the number of units depends on the concentration printed on the pharmacy label. Follow your prescriber's instructions and the pharmacy label, and ask the pharmacist. Compounded semaglutide is not FDA-approved, and FDA has received reports of dosing errors with it, some needing hospital care [3].
Missed doses and changing your day
- Remembered a missed dose within 5 days of the missed day? Take it then. More than 5 days late, skip it and use your regular day [1].
- A new weekly day is fine when at least 2 days (more than 48 hours) separate the two doses [1].
- Insulin users give Ozempic as a separate injection; never mix the two. Same body area is fine, but not right next to each other [1].
What to expect when you start
The 0.25 mg starting dose is there to help your body adjust. Digestive effects are the most common: nausea occurred in 15.8% of people on 0.5 mg and 20.3% on 1 mg in the label trials, vs 6.1% on placebo [1]. Diarrhea (8.5% to 8.8%) and constipation (3.1% to 5.0%) were also reported [1], so bowel habits may change in either direction. Call your prescriber if you cannot keep fluids down, because dehydration has led to kidney injury [1]. The full list is on Ozempic side effects, injection steps are on how to use Ozempic, and what each dose did in trials is on Ozempic results.
Dose safety
Never take more than one weekly dose to catch up. With insulin or a sulfonylurea, your prescriber may lower those doses to reduce the risk of low blood sugar [1]. Stop and call your prescriber for severe, lasting stomach pain [1]. Do not use Ozempic with a personal or family history of medullary thyroid carcinoma or MEN 2 [1]. Call 911 for trouble breathing or throat swelling.
What each dose costs
NovoCare's self-pay price is $349 a month for 0.25, 0.5 or 1 mg and $499 a month for 2 mg, with new patients paying $199 for the first two fills of 0.25 or 0.5 mg through December 31, 2026 (checked October 17, 2026) [4]. With commercial coverage, the savings offer can bring the cost to $25 a month, with savings capped at $100 a month [4]. See Ozempic cost and Ozempic vs Mounjaro for alternatives.
