Fountain Life review: cost, locations and complaints

Fountain Life does not publish its membership prices; a third-party site, Keep Health, reported APEX at $19,500 a year and CORE at $6,500 per person when bought in groups of 10 [1]. We could not confirm those figures with the company, so treat them as reported prices, not a current price list.

Medical review: pending. A licensed clinician has not yet reviewed this page.

Fountain Life is a US longevity clinic company that sells annual memberships (CORE, APEX, APEX Family and Executive Health) for preventive testing and care.

Our verdict

Clean

Provisional clean rating. Fountain Life publishes no prices; a third party reported $19,500 a year for APEX. We found no FDA warning letter, and we have not yet confirmed its BBB record.

FDA record
No FDA warning letter to Fountain Life among the 68 letters in our tracker (Dec 2024 to Sept 2026).
BBB
Not yet confirmed. We will add the BBB rating and complaint count after we check it.
Best for
People near Dallas, New York, Orlando, Naples or Houston who want one clinic to run a high-cost annual testing program and will get the price and contents in writing first.
Watch out
No published prices; the only figures we have are from a third party (APEX $19,500 a year, CORE $6,500 each in groups of 10). We could not confirm what each tier includes. If a program includes whole-body MRI, the ACR does not recommend it for people without symptoms; if it includes Galleri, that test is not FDA-approved.

Fountain Life figures are low confidence. Ask the company for a written quote.

Before paying, ask Fountain Life for a written list of what your tier includes, which tests or visits are billed separately, whether any part is billed to insurance, and the cancellation and refund terms. We could not confirm any of those points from public pages.

Programs and locations

Fountain Life names four programs: CORE, APEX, APEX Family and Executive Health [2]. Its listed locations are Dallas, New York, Orlando, Naples and Houston, with Miami and Los Angeles described as coming [2] [1]. For concierge and executive options in the same cities, see executive physicals and executive physical cost.

What we could not confirm

  • Current prices from Fountain Life itself.
  • Which tests, scans and visits each tier includes.
  • Refund and cancellation terms.
  • The company's BBB rating and complaint history.

Because the price, the core fact on this page, is not confirmed, we mark this review as incomplete and will update it when we can read Fountain Life's own terms.

Evidence limits that apply to clinic programs like this

We do not know which tests Fountain Life includes in each tier, so these points are conditional. If a program includes whole-body MRI, the American College of Radiology does not recommend that screening for people without symptoms, risk factors or family history [4]. If it includes the Galleri cancer blood test, Galleri was not FDA-approved as of Oct 17, 2026 [5] and had a 61.6% positive predictive value in PATHFINDER 2 [6]. If it reports a biological age, results for the same sample can differ significantly between companies [7].

Record check

No letter to Fountain Life appears in our FDA warning letter tracker (68 letters, December 2024 to September 2026). Fountain Life is a separate company from Fountain TRT; see our Fountain TRT review for that one.

BBB record not yet confirmed

We have not yet confirmed Fountain Life's BBB rating or complaint history. The clean rating is provisional.

Who might consider it, and the a la carte route

A clinic membership bundles scheduling and interpretation. If you mainly want the tests, here is what common longevity tests cost when bought one at a time. Smaller programs with published prices include Lifeforce, which tests blood every three months with coaching [14], and the one-hour Neko Health scan in New York [15]. Single at-home lab kits are sold by Everlywell [16].
