Concierge medicine is a model of primary care in which patients pay a periodic fee for better access, longer visits and coordination, while covered care may still be billed to insurance.
Ohio's direct primary care law
Ohio passed direct primary care provisions in HB 166 of the 133rd General Assembly in 2019, at Ohio Revised Code 3901.95 [1]. DPC Frontier lists Ohio among 32 states with a DPC law [6]. That matters in Cincinnati, where the median household income is $52,909 [5]: a direct primary care plan at the AAFP's typical $50 to $100 a month [7] costs far less than most concierge retainers.

Medicare and HSA rules
Medicare does not cover concierge membership fees, and a concierge fee cannot include extra charges for Medicare-covered services when the doctor accepts assignment [10]. For people with HSAs, IRS Notice 2026-5 allows qualifying DPC fees up to $150 a month per individual to be paid from the account starting in 2026; arrangements that bill insurance are excluded [11]. Employer-paid DPC fees cannot be reimbursed from the HSA [11].
Executive physicals and referral rules
If you want a one-time workup instead of a membership, see executive physical cost. Ohio's medical board can discipline physicians who split fees or accept anything of value for referring patients, unless the law allows it [12]. Check any doctor's license with the State Medical Board of Ohio [13].
