Medical weight loss in Cincinnati: semaglutide, tirzepatide and costs

Most Cincinnati adults who want semaglutide for weight loss pay cash or use a job-based plan, because Ohio Medicaid does not cover GLP-1s for obesity [1]. On Oct 17, 2026, Novo Nordisk charged $349 a month for Wegovy (semaglutide) pens and Lilly charged $299 to $449 for Zepbound (tirzepatide) [2] [3].

Cincinnati and Hamilton County at a glance

Hamilton County's adult obesity rate was 35.6% in CDC's 2023 PLACES model, and 13.2% of adults there reported a diabetes diagnosis [4]. Cincinnati itself counted 314,367 residents in mid-2025, and its median household earns $52,909 [5]. That income shapes everything else here. Twelve months of Wegovy pens at $349 comes to $4,188 [2], close to 8% of what the median Cincinnati household brings in.

Semaglutide prices for Cincinnati cash buyers

Wegovy is the semaglutide brand labeled for weight and heart risk reduction [6]; Ozempic is the diabetes version [7]. Novo sells Wegovy two ways. The weekly pen is $349 for every standard strength from 0.25 to 2.4 mg, and a first-time buyer pays $199 for each of the opening 0.25 and 0.5 mg boxes through December 31, 2026 [2]. The daily tablet is cheaper at the start: 1.5 mg is $149, 4 mg is $199, and 9 or 25 mg is $299 [2].

Lilly's two weight drugs sit in a similar band. Its Foundayo tablet runs $149 to $299 depending on strength [8]. Zepbound is $299 at 2.5 mg, $399 at 5 mg and $449 from 7.5 through 15 mg if each refill is on time, and Walmart pharmacies can hand it over [3]. When the two injectables went head to head in SURMOUNT-5, average loss at 72 weeks was 20.2% on tirzepatide and 13.7% on semaglutide [9]. Who pays in Ohio

Ohio Medicaid members are on their own for weight-loss GLP-1s: KFF's January 2026 roster of 13 covering states leaves Ohio off [1]. Older Cincinnatians on a Part D plan have one opening. The temporary Medicare GLP-1 Bridge charges $50 a month for Wegovy, Foundayo or the Zepbound KwikPen until December 31, 2027, for people who meet its BMI and health rules [10] [11]. Our Medicare GLP-1 page lists those rules, and GLP-1 cost compares every channel.

Choosing a Cincinnati prescriber

  • Run the prescriber's name through the State Medical Board of Ohio license search [12].
  • Under Ohio Revised Code 4731.22, the board may discipline a doctor who splits fees for referrals or takes anything of value for sending a patient somewhere [13]. Ask how a clinic finds its patients if the ads feel pushy.
  • Ask whether you will get brand Wegovy or a compounded copy. FDA has not approved compounded semaglutide and does not check its safety, effectiveness or quality [14]. Our compounded semaglutide page has more.
  • Look up any online brand in our FDA warning letter tracker.

Before the first Cincinnati fill

Skip semaglutide and tirzepatide if you or a close relative has had medullary thyroid cancer or MEN 2. Belly pain that is severe and does not let up can be pancreatitis, so stop and get seen. Insulin or a sulfonylurea plus a GLP-1 can push blood sugar too low. Wegovy should be stopped at least 2 months before a planned pregnancy [6] [15]. A swollen face or trouble breathing is a 911 call.
