What is available in Chicago
Northwestern Medicine lists a concierge medicine program, described as primary care from dedicated physicians, and an executive health program offering comprehensive care in one day, under its Signature Services page. Neither shows a price [1]. National networks such as MDVIP set fees by physician and locale [5], and direct primary care clinics post monthly prices.
Chicago's median household income was $77,902 in 2020 to 2024 [3]. A concierge retainer of a few thousand dollars a year is a large share of that, so the value question is real. Our is concierge medicine worth it page lays out the trade-offs.

Concierge, DPC or a hybrid membership
The American Academy of Family Physicians draws the line this way: direct primary care charges low monthly fees and generally does not bill insurance, while concierge practices use annual contracts with higher fees and may bill insurance for covered services [11]. Illinois has no law that specifically defines direct primary care, according to DPC Frontier [12]. Compare models on concierge vs direct primary care and browse DPC practices.
Medicare and HSA rules
Medicare does not cover concierge membership fees, and a doctor who accepts assignment cannot use the fee to charge extra for Medicare-covered services [4]. Since January 1, 2026, a direct primary care arrangement costing no more than $150 a month for one person can be paid from an HSA, but arrangements that also bill insurance do not qualify [13]. KerixHealth's $199 adult fee exceeds that cap [2], and Notice 2026-5 says fees over the cap make the enrollee ineligible to contribute to an HSA while enrolled [13].
Executive physicals and longevity clinics
Searches for longevity clinics and functional medicine doctors in Chicago often lead to cash-pay testing packages. Northwestern's executive health program is the one hospital-run option we could confirm in research [1].
