Mounjaro results are the blood sugar and body weight changes measured in clinical trials of Mounjaro (tirzepatide) described in its FDA label.
How Mounjaro works
Mounjaro contains tirzepatide, which activates two hormone receptors, GIP and GLP-1 [1]. It boosts insulin release and lowers glucagon when blood sugar is high, and it delays stomach emptying [1]. The label's main goal is lower blood sugar; weight change was recorded in the trials as a secondary result. For a comparison with single-hormone medicines, see Ozempic vs Mounjaro.
Is Mounjaro approved for weight loss?
No. Mounjaro is approved for blood sugar control in adults and children 10 and older with type 2 diabetes, and to lower major cardiovascular events in high-risk adults with type 2 diabetes. The use in children was added to the label in December 2025 and the heart use in August 2026 [1]. Using Mounjaro only to lose weight is off-label. Zepbound, the same medicine, was approved for chronic weight management on November 8, 2023 [4]. Mounjaro was first approved on May 13, 2022 [3]. The Mounjaro vs Zepbound page explains the label differences.
What the Mounjaro trials found
| Trial | Dose | A1C change | Weight change |
|---|---|---|---|
| SURPASS-1 (vs placebo) | Placebo | -0.1 | -1.0 kg |
| SURPASS-1 | 5 mg | -1.8 | -6.3 kg |
| SURPASS-1 | 10 mg | -1.7 | -7.0 kg |
| SURPASS-1 | 15 mg | -1.7 | -7.8 kg |
| SURPASS-2 (vs semaglutide 1 mg) | Semaglutide 1 mg | -1.9 | Not shown here |
| SURPASS-2 | 5 / 10 / 15 mg | -2.0 / -2.2 / -2.3 | Not shown here |
In SURPASS-2, 79% of people on semaglutide 1 mg reached an A1C under 7%, vs 82% to 86% on Mounjaro [1]. Fasting glucose fell by 49 mg/dL on semaglutide and 55 to 60 mg/dL on Mounjaro [1].
What tirzepatide did in weight-management trials (Zepbound)
Zepbound's label trials show what tirzepatide does when studied for weight. In adults without diabetes (SURMOUNT-1), average weight change at 72 weeks was -15.0% on 5 mg, -19.5% on 10 mg and -20.9% on 15 mg, vs -3.1% on placebo [2]. In adults with type 2 diabetes, it was -12.8% on 10 mg and -14.7% on 15 mg, vs -3.2% on placebo [2]. People with diabetes lost less on average. In SURMOUNT-5, tirzepatide averaged -20.2% vs -13.7% on semaglutide at 72 weeks [5]. See Zepbound results.
How long Mounjaro takes to work
The label does not name a start date. Everyone begins on 2.5 mg once weekly, and the dose can rise by 2.5 mg after at least 4 weeks on the current dose [1]; the steps are on Mounjaro dosage. Trials measured results at 40 weeks for blood sugar and 72 weeks for weight [1] [2]. Appetite timing is not described. Tirzepatide's half-life is about 5 days [1], which is what makes once-weekly dosing work.
Why some people lose less, or gain
Trial averages hide a range. In SURMOUNT-1, 85% to 91% of people lost at least 5% of their weight, which means roughly 1 in 10 did not [6]. In Zepbound's trials, adults with type 2 diabetes lost less on average than adults without it (14.7% vs 20.9% on 15 mg) [2]. Dose matters too: the Zepbound label calls 2.5 mg a starting dose that is not meant for maintenance [7]. Mounjaro's label instead says to raise the dose when more blood sugar control is needed [1]. Insulin and some other medicines can also affect weight. If you are not losing weight, ask your prescriber whether your dose, your medicine or your diagnosis fits your goal. Trial results are averages, not promises.
Is Mounjaro safe, and how long do people stay on it?
Mounjaro carries a boxed warning about thyroid C-cell tumors seen in rats, plus warnings on pancreatitis, gallbladder disease, kidney injury and low blood sugar [1]; see Mounjaro side effects. There is no maximum treatment length on the label. Weight tends to return when tirzepatide ends: in SURMOUNT-4, the group moved to placebo after 36 weeks gained back 14.0% of body weight over the next year, while the group that continued lost a further 5.5% [8]. On price, Lilly's list figure is $1,112.16 per 28-day box of four pens [9]. Its savings card can bring that down to $25 for people whose commercial plan covers Mounjaro, or to as low as $499 a month for people whose commercial plan does not; it cannot be used by the uninsured or by government plan members, and it expires December 31, 2026 [10].
