OrderlyMeds names SmartPharmaRx as one of its pharmacies [2], and we found no FDA recall for it [3].
SmartPharmaRX is a compounding pharmacy that says it holds a 503A pharmacy license and PCAB accreditation.
What SmartPharmaRX says about itself
- It calls itself a "503A Licensed Pharmacy" and an "Accredited PCAB Compounding Pharmacy" [1].
- It says it follows USP chapters 795, 797 and 800 and that batches are tested for sterility, potency and endotoxins [1].
- It says it partners with healthcare providers nationwide [1].
- Its site gives no street address, no home state and no list of state licenses [1].
The site's careers link goes to a job board whose web address includes "orderly-wellness" [1]. We did not find a primary source that says who owns SmartPharmaRX, so we draw no conclusion from that link.
503A or 503B?
SmartPharmaRX describes itself as 503A [1] and is not on FDA's registered outsourcing facility list, checked Oct 24, 2026 [4].
License check
Without a home state we could not look up a license. If you get a SmartPharmaRX label, use the address printed on it to find the right state board through FDA's BeSafeRx list [5].
FDA record
We found no recall records for "SmartPharma" in FDA's openFDA recall data on Oct 24, 2026 [3], and no FDA warning letter in our search of FDA's site. FDA's compounding actions page is long, and our tool could not read the whole list, so we cannot rule out an entry there [6].
Which providers we review use it
OrderlyMeds says it "exclusively works with pharmacies based in the United States, including SmartPharmaRx, Casa, and PerfectRx/PerfectionRx" [2]. FDA sent OrderlyMeds a warning letter on Jun 8, 2026 [7]. Read our OrderlyMeds review, and see Casa Pharma RX and PerfectRx.
What to do as a patient
Any drug SmartPharmaRX compounds is not FDA-approved. FDA says it does not check the safety, effectiveness or quality of compounded drugs before they are sold [8]. That holds for compounded semaglutide and compounded tirzepatide too.
- Read the label. The pharmacy name and address on the vial or box should match a pharmacy you can look up. FDA lists a pharmacy name on the label that you think might be fraudulent as a red flag [9].
- Check the license with the state board. FDA's BeSafeRx page links every state board of pharmacy license search [5]. Look up the pharmacy in its home state and in the state where you live.
- Check the website with NABP. The National Association of Boards of Pharmacy runs a Safe Site Search on Safe.Pharmacy and says a safe online pharmacy accepts only valid prescriptions, is licensed where it is located and where you live, and has a pharmacist you can reach [10].
- Never buy research-use-only vials. They are not legal for human use and are not pharmacy products.
- Report problems. FDA asks people to report side effects or quality problems with any medicine to MedWatch [9]. For a medical emergency, call 911.
Other pharmacies are on all the pharmacies we checked.
