In the 76-week SYNCHRONIZE-1 trial, adults without diabetes lost up to an average of 16.6% of body weight, vs 3.2% on placebo, if they stayed on treatment [2].
Survodutide is an investigational once-weekly injection from Boehringer Ingelheim, licensed from Zealand Pharma, that acts on both the glucagon and GLP-1 receptors [2] [3].
When will survodutide be available?
Not before FDA approves it, and no filing has been announced. Boehringer Ingelheim has reported two phase 3 weight trials, SYNCHRONIZE-1 in June 2026 and SYNCHRONIZE-2 in October 2026 [2] [1]. Neither announcement gave a date for regulatory submissions. The next result Zealand named is SYNCHRONIZE-CVOT, a heart outcomes trial, expected later in 2026 [1].
Once a company files a complete application, FDA says its review takes 6 to 10 months [4]. Until then there is no release date. Follow it in our GLP-1 pipeline tracker. You can also read where CagriSema, MariTide and amycretin stand.
Survodutide phase 3 results
Both trials gave survodutide 3.6 mg or 6.0 mg weekly, or placebo, for 76 weeks [2] [1]. The headline figures use the efficacy estimand, which assumes people stayed on treatment. Counting everyone who started, the figures are smaller.
| Trial | Who was in it | Survodutide | Placebo | Lost at least 5% | Source |
|---|---|---|---|---|---|
| SYNCHRONIZE-1 | 725 adults with obesity or overweight, without type 2 diabetes | Up to -16.6% (efficacy estimand) | -3.2% | Up to 85.1% vs 38.8% | [2] [3] |
| SYNCHRONIZE-2 | 755 adults with obesity or overweight and type 2 diabetes | Up to -13.1% (efficacy estimand); -8.2% and -9.8% counting everyone | -3.1%; -3.9% counting everyone | Up to 79.3% vs 32.7% | [1] [5] |
In SYNCHRONIZE-2, A1C fell by up to 1.21 points from a starting average of 7.4%, vs 0.03 on placebo, and up to 29.5% reached an A1C under 5.7%, vs 4.0% [1]. In a body composition substudy of SYNCHRONIZE-1, visceral fat fell by up to 34.0% and liver fat by up to 63.1%, and lean mass made up no more than 10.8% of the tissue lost at the highest dose [2].
Survodutide side effects
The most common side effects were nausea, vomiting, diarrhea and constipation, mostly mild to moderate and mostly during dose increases [2]. Many people stopped: 19% stopped because of stomach side effects in SYNCHRONIZE-1, vs 2.9% on placebo [2], and 18% in SYNCHRONIZE-2, vs 1.2% [1]. Boehringer says it plans new dosing guidance, and a phase 3b study, SYNCHRONIZE-START, will focus on tolerability [2].

How survodutide works
Survodutide is a dual agonist: it activates the GLP-1 receptor, like Wegovy, and the glucagon receptor [3]. Boehringer is also testing survodutide in MASH, a fatty liver disease, and it holds an FDA Breakthrough Therapy designation for MASH with liver scarring [5]. For background, see how GLP-1 drugs work.
Survodutide vs Wegovy and Zepbound
No head-to-head trial has been reported. The table puts each drug's main obesity trial in adults without diabetes side by side. The trials differ, so this is not a ranking.
| Survodutide | Wegovy (semaglutide) | Zepbound (tirzepatide) | |
|---|---|---|---|
| FDA status | Not approved [1] | Approved | Approved |
| Receptors | Glucagon and GLP-1 [3] | GLP-1 | GIP and GLP-1 [6] |
| Main trial | SYNCHRONIZE-1, 76 weeks [2] | STEP 1, 68 weeks [7] | SURMOUNT-1, 72 weeks [8] |
| Average weight change | Up to -16.6% vs -3.2%, efficacy estimand [2] | -14.9% on 2.4 mg vs -2.4% [7] | -20.9% on 15 mg vs -3.1% [8] |

How to get survodutide legally
Only in a clinical trial. Search open studies on ClinicalTrials.gov. FDA says it has warned companies selling survodutide and other GLP-1 products labeled "for research purposes" or "not for human consumption," and that those products have unknown quality and may be harmful [11]. See research-use-only GLP-1s and the approved options in online GLP-1 programs.
How we check
Results come from Zealand Pharma's company announcements, which report Boehringer's data, and from trade coverage of the published trial. We name the estimand for each number. When Boehringer files or FDA acts, this page and the pipeline tracker change.
Questions people ask
When will survodutide be available?
Is survodutide FDA approved?
No. Zealand Pharma's Oct 1, 2026 announcement says survodutide is investigational and has not been approved for use [1].
How much weight do people lose on survodutide?
What are the side effects of survodutide?
Mostly nausea, vomiting, diarrhea and constipation. In SYNCHRONIZE-1, 19% stopped because of stomach side effects vs 2.9% on placebo [2].
Is survodutide better than Zepbound?
Can I buy survodutide online?
Not legally. FDA has warned companies selling survodutide labeled for research, and says such products have unknown quality and may be harmful [11]. The legal route is a clinical trial.
