How we check

What happens when you stop tirzepatide

The tirzepatide label does not set a number of days to stop before surgery. It says food or liquid getting into the lungs has been reported during anesthesia or deep sedation, and that you must tell your providers you take it before any planned procedure [1]. Your surgeon and anesthesia team decide the timing.

Medical review: pending. A licensed clinician has not yet reviewed this page.

Stopping tirzepatide means ending weekly Zepbound, Mounjaro or compounded tirzepatide injections, whether for surgery, pregnancy, side effects, cost or a planned end to treatment.

Before surgery
No set stop interval in the label; tell every provider [1]
Weight after stopping
Regained 14.0% of body weight over 52 weeks on placebo in SURMOUNT-4 [2]
If you stay on it
Further 5.5% loss over the same 52 weeks [2]
Kept most of the loss
89.5% who continued vs 16.6% who stopped kept at least 80% [2]
Half-life
About 5 to 6 days [1]

Before surgery, a colonoscopy or sedation

The warning is about pulmonary aspiration: stomach contents entering the lungs while you are under general anesthesia or deep sedation. Tirzepatide slows stomach emptying [1], and aspiration has been reported in people taking it. The label gives no stop interval or other step to reduce the risk; it tells patients to inform their providers before any planned surgery or procedure [1].

A colonoscopy done with deep sedation falls under the same warning. Ask the team doing the procedure, not the internet, when your last dose should be and whether your diet before it should change. The label also gives no rule on when to restart afterward; your surgeon or prescriber sets that.

Say it out loud

Tell your surgeon, anesthesia team and endoscopy team that you take tirzepatide, including compounded tirzepatide, before the day of the procedure [3]. Call 911 for trouble breathing at any time.

What happens to your weight when you stop

SURMOUNT-4 is the clearest test. Everyone took tirzepatide openly for 36 weeks and lost an average of 20.9%. Then people were randomly assigned to keep taking it or switch to placebo for another 52 weeks [2].

SURMOUNT-4: continuing vs stopping tirzepatide
MeasureKept taking tirzepatideSwitched to placebo
Weight change, weeks 36 to 88-5.5%+14.0%
Total weight change, weeks 0 to 88-25.3%-9.9%

Averages from the SURMOUNT-4 trial abstract [2].

So yes, on average people regained much of the weight in the year after stopping, though the placebo group still ended 9.9% below where it started [2]. Our sources include no trial of a specific plan for keeping weight off after tirzepatide ends.

Cold turkey or taper?

The label gives no taper schedule. The drug leaves the body gradually on its own: its half-life is about 5 to 6 days, so levels fall by about half every 5 to 6 days after the last shot [1]. Whether to step down first is a decision for you and your prescriber. If you stop and later restart, ask what dose to restart at rather than going back to your old dose.

How long can you stay on it?

Zepbound is approved to reduce excess weight and keep it off long term [4]. The longest trial in our sources ran 88 weeks [2], and tirzepatide has been on the US market since Mounjaro's approval on May 13, 2022 [5]. Long-term unknowns remain: the boxed warning says it is unknown whether the thyroid C-cell tumors seen in rats occur in people [1]. Full risks are on tirzepatide side effects.

Does tirzepatide stop working?

In SURMOUNT-4, people who kept taking it continued to lose weight on average through week 88 [2]. Our sources hold no data on the drug losing its effect over time. A plateau can have many causes, including still being on a low dose; the label allows 2.5 mg steps after at least 4 weeks, up to 15 mg [4]. See tirzepatide results and tirzepatide dosage.

Stopping because of side effects

In the pooled Zepbound weight trials, 4.8% to 6.7% of people stopped for side effects, vs 3.4% on placebo [1]. The label says to stop if pancreatitis is suspected and to stop and get medical help for a serious allergic reaction [1].

  • Hair loss: reported by 4% to 5% vs 1% on placebo, more often in women [1]. The label offers no treatment for it. Talk to your prescriber before stopping.
  • Diarrhea: drink fluids and call your prescriber if it does not go away, since fluid loss can harm the kidneys [3].

Stopping before pregnancy

Zepbound should be stopped once pregnancy is recognized [1]. The tirzepatide labels give no set interval to stop before trying to conceive; semaglutide labels say at least 2 months [6]. Plan the timing with your prescriber, and note that tirzepatide can make birth control pills less reliable [7]. More on tirzepatide interactions.

If cost is the reason

Many people stop because of price. LillyDirect lists Zepbound at $299 to $449 a month depending on dose, with the $449 price for 7.5 to 15 mg only when refilled within 45 days, checked October 17, 2026 [8]. Compare options on tirzepatide cost and switching GLP-1s. Lower "microdoses" are sometimes marketed for maintenance; read microdosing tirzepatide for what the evidence shows.

Questions people ask

When should you stop tirzepatide before surgery?

The label sets no number of days. It warns that aspiration has been reported under anesthesia or deep sedation and tells patients to inform providers before any planned procedure [1]. Your surgical team decides.

What happens when you stop taking tirzepatide?

In SURMOUNT-4, people switched to placebo after 36 weeks regained an average of 14.0% of body weight over the next 52 weeks, while those who continued lost a further 5.5% [2].

Can you stop tirzepatide cold turkey?

The label gives no taper schedule. The drug clears gradually, with a half-life of about 5 to 6 days [1]. Decide how to stop with your prescriber.

Do you have to take tirzepatide forever?

No rule requires it, but in SURMOUNT-4 only 16.6% of people who stopped kept at least 80% of their weight loss, vs 89.5% who continued [2]. Zepbound is approved for long-term weight maintenance [4].

How long can you stay on tirzepatide?

Zepbound is approved for long-term use to keep weight off [4]. The longest trial in our sources, SURMOUNT-4, followed people for 88 weeks [2].

Does tirzepatide stop working over time?

In SURMOUNT-4, people who kept taking tirzepatide continued to lose weight on average through week 88, reaching 25.3% total loss [2].

When should I stop tirzepatide before a colonoscopy?

The label gives no set interval. A colonoscopy with deep sedation falls under the aspiration warning, so tell the endoscopy team you take tirzepatide and follow their instructions [1].

When should you stop tirzepatide before pregnancy?

Tirzepatide labels give no planned-pregnancy interval; Zepbound should be stopped once pregnancy is recognized [1]. Semaglutide labels, by contrast, say at least 2 months before [6].

How do I stop hair loss from tirzepatide?

The label lists hair loss (4% to 5% vs 1% on placebo, and 7.1% of women) but gives no treatment for it [1]. Ask your prescriber before stopping or changing your dose.

What are the long-term side effects of tirzepatide?

Trials ran up to 88 weeks [2]. The boxed warning notes it is unknown whether the thyroid C-cell tumors seen in rats occur in people [1].
