Stopping Wegovy means ending the weekly pen or the daily pill, after which semaglutide leaves the body slowly and its effects on appetite and weight wear off.
- Weight back within a year
- Roughly two-thirds of what was lost [1]
- Net change a year after stopping
- Down 5.6%, vs 0.1% for placebo [1]
- How long the label allows
- Long-term use; no end date set [2]
- Coming back after 2+ missed weeks
- A lower restart dose may be needed [2]
- Trial dropouts over side effects
- 6.8% on Wegovy, 3.2% on placebo [2]
The year after Wegovy: what the extension study found
| Time point | Weight change from start |
|---|---|
| Week 68, when treatment ended | Down 17.3% |
| Week 120, one year off the drug | Down 5.6% (placebo down 0.1%) |
| Regained in that year | 11.6 percentage points, about two-thirds |
Heart and metabolic markers that had improved on treatment largely drifted back as well [1]. The label explains why the change is gradual: semaglutide has a long half-life, which is why it asks people to stop 2 months before a planned pregnancy [2]. When appetite comes back is not stated.
Is Wegovy a lifelong drug?
The label does not require lifelong use. It approves Wegovy for losing excess weight and keeping it off over the long term [2], and in the SELECT trial the median time on treatment was 37.3 months [2]. Stopping after 1 month or 3 months is your choice to make with your prescriber; the extension study shows the likely trade-off.
Research on very long use is still under way. The boxed warning about thyroid C-cell tumors, and the rest of the label warnings, stay in force for as long as you take it [2]. Cost over years matters too; see Wegovy cost.
Quitting all at once or stepping down
Wegovy's label tells prescribers how to climb the dose but gives no step-down plan, and we found no withdrawal syndrome described in the sections we read. Have the conversation before your last dose, above all if you use insulin or a sulfonylurea: the label already flags those for dose changes alongside Wegovy [2].
Taking a two-week break
Two weekly doses missed back to back is where the label's restart rule kicks in. Contact your provider, who may start you again at a lower step [2]. Going straight back to your previous dose after a break is not in the label. One missed pen dose falls under the 48-hour rule; a missed tablet is simply skipped [2].
Surgery, sedation and pregnancy
Procedures. During general anesthesia or deep sedation, stomach contents can be inhaled into the lungs. The label says current data cannot tell whether pausing Wegovy or changing fasting rules lowers that risk, and it asks patients to let their care team know they take it [2]. The surgical team decides. More in Wegovy interactions.
Trying to conceive. Come off Wegovy no later than 2 months before you plan to get pregnant. If a pregnancy is confirmed while you are taking it, stop then [2].
Fixing side effects instead of quitting
Side effects ended treatment for 6.8% of trial adults on Wegovy 2.4 mg compared with 3.2% on placebo, with nausea, vomiting and diarrhea at the top [2]. One option worth asking about first: the label lets a prescriber put off the next dose increase by 4 weeks [2].
- Nausea and throwing up: reported most while the dose was climbing [2]. Vomiting that will not stop can dry you out and hurt the kidneys, so call that day [2].
- Diarrhea: 30% vs 16% on placebo, with the same fluid-loss risk [2].
- Lightheadedness: 8% vs 4% [2]. With type 2 diabetes, it may be a low blood sugar sign [2].
- Thinning hair: 3.3% vs 1% [2]. Whether it grows back after stopping is not covered.
- Feeling cold or period changes: not among the label reactions we found. Report them anyway.

Weight loss has stalled: options to raise
Wegovy's label does not describe the drug losing its effect over time. Some people respond less from the start: 13.6% in STEP 1 did not get to 5% loss [3]. A prescriber may look at moving to 7.2 mg, which the label allows for some adults after at least 4 weeks on 2.4 mg [2], or at changing drugs. Questions people ask
What happens when you stop taking Wegovy?
Most people regain a good part of the weight. A year after stopping semaglutide 2.4 mg in the STEP 1 extension, 11.6 of the 17.3 percentage points lost had returned [1].
Can you stop Wegovy cold turkey?
The label has no step-down schedule, and we did not find a withdrawal syndrome in it. Speak to your prescriber first, particularly if you use insulin or a sulfonylurea [2].
Do you have to take Wegovy forever?
What happens if you stop Wegovy for 2 weeks?
Two missed weekly doses in a row trigger the label's restart rule: call your provider, who may restart you at a lower dose [2].
Do you gain weight back after stopping Wegovy?
On average, yes. In the STEP 1 extension the group went from 17.3% down to a net 5.6% down one year after stopping, vs 0.1% for placebo [1].
When should you stop Wegovy before surgery?
The label gives no hold time. It says data are too limited to recommend pausing, and asks you to tell your providers before any procedure [2].
How long does Wegovy stay in your system after stopping?
The label gives no number of days. It describes the half-life as long and says to stop at least 2 months before a planned pregnancy [2].
How do I stop nausea from Wegovy without quitting?
Ask whether your next dose increase can wait. The label lets a prescriber delay it by 4 weeks when a step is not tolerated [2].
What are the long-term effects of Wegovy?
Median exposure in the SELECT trial was 37.3 months. The boxed thyroid C-cell tumor warning and the other label warnings apply for the whole time you take it [2].
