A semaglutide interaction is a way another medicine, or a health condition, changes the safety or effect of semaglutide, or semaglutide changes theirs.
Medicines the label flags
- Other semaglutide or GLP-1 drugs. Using Wegovy with other semaglutide products or any other GLP-1 receptor agonist is not recommended [1]. That includes stacking a compounded semaglutide on a branded one. Switching between drugs is covered in switching GLP-1 medicines.
- Insulin and sulfonylureas. These raise the risk of low blood sugar, including severe episodes; the label says to consider lowering their dose when semaglutide starts [1].
- Pills taken by mouth. Semaglutide delays stomach emptying and may affect how oral drugs are absorbed. With Wegovy tablets, levothyroxine exposure rose 33%, and the label advises monitoring oral drugs with a narrow safety margin [1].
- Other pills with Wegovy tablets. Wait at least 30 minutes after the tablet before taking other oral medicines [1].
Phentermine, metformin, ibuprofen and Tylenol
None of these four is named in the semaglutide label facts we reviewed. That means the label gives no specific guidance, not that the pairing has been studied or is safe. Your prescriber and pharmacist should check your full list.
Is metformin a semaglutide? No. Metformin is a separate diabetes medicine. Semaglutide is a GLP-1 receptor agonist [1]. Birth control pills are oral medicines, so the general label caution about oral drug absorption applies; we did not find a statement specific to contraceptives in our sources [1].
Pregnancy, fertility and breastfeeding
Semaglutide may cause fetal harm. For weight or heart use, Wegovy should be stopped when pregnancy is recognized, and the label says to stop at least 2 months before a planned pregnancy because of semaglutide's long half-life [1]. The Ozempic label says to use it in pregnancy only if the benefit justifies the risk, with the same 2-month advice [2].
Breastfeeding is not recommended with Wegovy tablets. For the injection, there are no human milk data [1]. The label facts we reviewed do not address fertility; ask your prescriber if you are trying to conceive.
Heart, blood pressure and kidneys
Heart. Wegovy is approved to lower the risk of major heart events in adults with established heart disease and obesity or overweight [1]. In the SELECT trial behind that approval, major heart events occurred in 6.5% on Wegovy vs 8% on placebo [3]. Resting heart rate rose 1 to 4 beats per minute on average [1].
Blood pressure. We did not find blood pressure figures in the label sections we reviewed, so we cannot say whether semaglutide raises or lowers it. If you take blood pressure medicine, keep monitoring as your prescriber advises.
Kidneys. The label warns of acute kidney injury from dehydration after vomiting or diarrhea [1]. Ozempic is approved to reduce the risk of kidney decline, kidney failure and heart death in adults with type 2 diabetes and chronic kidney disease [2].
Liver, thyroid and diabetes
Liver. Liver injury is not among the Wegovy label's warnings (Sections 5.1 to 5.11) [1]. Wegovy injection has accelerated approval for noncirrhotic MASH, a fatty liver disease, with moderate to advanced fibrosis [1].
Thyroid. The boxed warning covers thyroid C-cell tumors in rodents, and semaglutide is not for people with a personal or family history of medullary thyroid carcinoma or MEN 2 [1]. If you take levothyroxine for an underactive thyroid, note the 33% exposure increase seen with the tablet [1].
Diabetes. Ozempic is approved for type 2 diabetes [2]. We found nothing in our sources showing semaglutide causes diabetes. PCOS, inflammation and insulin resistance are not labeled uses, and we did not find data on them in our sources.
Is semaglutide a hormone? The label describes it as a GLP-1 receptor agonist [1]: a manufactured drug that acts on that receptor.
Surgery and procedures
Semaglutide may raise the chance of food getting into the lungs during anesthesia or deep sedation. The label says data are not enough to recommend changing fasting or pausing the drug before procedures, and that patients should tell their providers before any planned surgery [1]. When to restart afterward is the surgical team's call. See stopping semaglutide for more.
Side effect details are in semaglutide side effects, food and alcohol questions in what to eat on semaglutide, the Ozempic version in Ozempic interactions, and the Wegovy-specific version of this page is Wegovy interactions.
Questions people ask
Can you take phentermine and semaglutide together?
The semaglutide label facts we checked do not name phentermine, so there is no label guidance. Only your prescriber can decide after reviewing your history [1].
What can you not take with semaglutide?
Other semaglutide products or other GLP-1 receptor agonists are not recommended with it. Insulin and sulfonylureas need extra care because of low blood sugar risk [1].
Can you take semaglutide while pregnant?
Wegovy may cause fetal harm and should be stopped when pregnancy is recognized; stop at least 2 months before a planned pregnancy [1].
Can you take semaglutide while breastfeeding?
Breastfeeding is not recommended with Wegovy tablets. For the injection, there are no human milk data [1].
Can you take metformin with semaglutide?
Metformin is not named in the label facts we reviewed. The label's specific caution is about insulin and sulfonylureas, which raise low blood sugar risk [1].
Does semaglutide cause liver damage?
Liver injury is not among the Wegovy label warnings, and Wegovy injection has accelerated approval for MASH with moderate to advanced fibrosis [1].
Does semaglutide affect birth control?
The label says semaglutide may affect absorption of oral drugs because it slows stomach emptying [1]. We found no contraceptive-specific statement; ask your prescriber.
Does semaglutide affect the kidneys?
When can I resume semaglutide after surgery?
The label does not set a restart time. It says data are not enough to recommend pausing before procedures and that you should tell your providers you take it [1].
