Microdosing semaglutide: doses, cost and evidence

Microdosing semaglutide means taking doses below the FDA label schedule or holding a low starting dose long term, and no FDA label includes it: Wegovy and Ozempic both start at 0.25 mg once weekly, and the lowest labeled Wegovy maintenance dose for adult weight reduction is 1.7 mg weekly [1][2]. We could not confirm any published trial of semaglutide microdosing, so there is no trial average to report for it.

Medical review: pending. A licensed clinician has not yet reviewed this page.

Semaglutide microdosing is an informal, off-label practice of taking weekly semaglutide doses smaller than the FDA-labeled doses, or staying on a starting dose instead of moving up to a labeled maintenance dose.

What a semaglutide microdose means

There is no medical or regulatory definition of a semaglutide microdose. Online programs use the word for plans that keep weekly doses at or below the starting doses, or that move up more slowly than the label. Programs selling microdose plans on Oct 17, 2026 included Noom [4], Found [5] and Remedy Meds [6]. Hims & Hers lists a microdosing plan for the brand-name Wegovy pill [7].

Noom's footnotes point to compounded drugs [4]. Compounded semaglutide is not FDA-approved, and FDA does not review compounded drugs for safety, effectiveness or quality [15]. Read our semaglutide guide for how the approved brands differ.

Wegovy shot0.25 mg weekly, weeks 1 to 40.5 mg weeks 5 to 8; 1 mg weeks 9 to 12; 1.7 mg weeks 13 to 161.7 mg or 2.4 mg weekly; up to 7.2 mg if 2.4 mg is tolerated for at least 4 weeks[1]
Wegovy pill1.5 mg daily, days 1 to 304 mg days 31 to 60; 9 mg days 61 to 9025 mg daily from day 91[1]
Ozempic (type 2 diabetes)0.25 mg weekly for 4 weeks0.5 mg, then increases after at least 4 weeks on the current dose0.5 mg, 1 mg or 2 mg weekly[2]
Doses below the starting dose do not appear on any semaglutide label.

The Wegovy label lists 0.25 mg, 0.5 mg and 1 mg only as escalation steps, not as maintenance doses [1]. It does give one built-in way to go slower: if an escalation dose is not tolerated, the prescriber can consider delaying the next step by 4 weeks [1]. See our semaglutide dosage guide and Wegovy dosage guide for the full schedules.

Ozempic is approved for type 2 diabetes, not weight loss [2]. Taking it for weight loss, at any dose, is off-label. Our Ozempic vs Wegovy comparison covers the differences.

Can you stay on a low dose of Wegovy?

Not according to the label. The Wegovy label lists 1.7 mg and 2.4 mg as the maintenance doses for adult weight reduction [1], so staying at 0.25 mg or 0.5 mg long term is outside it. A prescriber can still choose to hold a dose or slow the climb, and the label allows a 4-week delay when a step is not tolerated [1]. Ask your prescriber which dose they plan to keep you on, and why.

Does microdosing semaglutide work?

We could not confirm any randomized trial that tested semaglutide doses below the label schedule for weight loss. That means we cannot say how much weight people lose on a microdose, or how long it takes to lose 15 pounds on one. The trial numbers that exist are for full labeled doses. In the STEP 1 trial, adults on semaglutide 2.4 mg lost an average of 14.9% of body weight at 68 weeks, compared with 2.4% on placebo [8]. More results are on our semaglutide results page.

Lower doses still cause side effects. In Ozempic's diabetes trials, 15.8% of people on 0.5 mg and 20.3% on 1 mg reported nausea, compared with 6.1% on placebo [2]. See semaglutide side effects for the full list.

A lower brand-pen dose does not lower the price: NovoCare charges $349 a month for every standard Wegovy pen dose [3]. Noom's plan locks you into a 12-week subscription after the $99 start [4]. Hims & Hers adds a monthly membership on top of the drug [7], and we rate it Avoid because of a July 2026 lawsuit from the FTC, Utah and Los Angeles County over subscriptions, cancellation and health data sharing [10]. Compounded semaglutide microdoses

FDA has received 990 adverse event reports for compounded semaglutide as of May 31, 2026, and says the real number is likely higher because 503A pharmacies are not required to report [11]. FDA also reports dosing errors with compounded injectable semaglutide, some needing hospital care, when patients mismeasured doses or clinicians miscalculated them [11].

FDA declared the semaglutide shortage over on February 21, 2025 [12]. Since then, a 503A pharmacy may compound it only for an individual patient, and copies of an approved drug are barred unless the prescriber documents a change that makes a significant difference for that patient [12]. In a September 18, 2026 warning letter to Empower Pharmacy, FDA said that significant-difference notes that were missing or repeated word for word call the individual nature of those decisions into question [13]. Our compounded semaglutide guide covers the rules.

How many units is a microdose?

Units on a syringe depend on the concentration printed on the pharmacy label, so the same number of units can hold different amounts of drug from one vial to the next. Follow your prescriber's instructions and the pharmacy label, and ask the pharmacist to confirm your dose. We do not give unit conversions or mixing instructions.

Missed doses and stopping

For a Wegovy shot, take a missed dose as soon as possible if the next dose is more than 2 days (48 hours) away; otherwise skip it. If you miss 2 or more doses in a row, contact your provider, because the label says to restart escalation at a lower dose [1]. For Ozempic, take a missed dose within 5 days, or skip it after that [2].

There is no data on stopping a microdose. After stopping full-dose semaglutide in the STEP 1 extension, people regained 11.6 of the 17.3 percentage points they had lost within a year, and heart and metabolic gains largely went back toward baseline [14]. Read more on stopping semaglutide.

Safety at any dose

Semaglutide carries a boxed warning: it caused thyroid C-cell tumors in rodents, and it is not known whether it does in humans. Do not use it with a personal or family history of medullary thyroid carcinoma or MEN 2 [1]. Stop and call your prescriber for severe stomach pain that will not go away, which can be a sign of pancreatitis [1]. Low blood sugar is more likely if you also take insulin or a sulfonylurea [1]. Stop semaglutide at least 2 months before a planned pregnancy [1]. For signs of a serious allergic reaction, such as trouble breathing, call 911.

Questions people ask

What is considered a microdose of semaglutide?

There is no official definition. The FDA labels start Wegovy and Ozempic at 0.25 mg once weekly, and the lowest labeled Wegovy maintenance dose for adult weight reduction is 1.7 mg [1][2]. Any dose below 0.25 mg, or staying on a starting dose long term, is outside the label.

Can you microdose Ozempic?

Ozempic is approved for type 2 diabetes, not weight loss. Its label starts at 0.25 mg weekly for 4 weeks, then 0.5 mg, with maintenance doses of 0.5 mg, 1 mg or 2 mg weekly [2]. Microdosing Ozempic for weight loss is off-label on two counts: the use and the dose.

Does microdosing semaglutide work for weight loss?

We could not confirm any published trial of semaglutide microdosing as of Oct 17, 2026, so no average weight loss can be given. At the full 2.4 mg dose in the STEP 1 trial, adults lost an average of 14.9% of body weight at 68 weeks, compared with 2.4% on placebo [8].

How much does a semaglutide microdose cost?

Through NovoCare, the Wegovy pen costs $349 a month at every standard dose, with a $199 price for 0.25 or 0.5 mg on a new patient's first two fills through Dec 31, 2026 [3]. Noom's microdose program costs $99 to start, then $199 a month on a 12-week subscription, checked Oct 17, 2026 [4].

How many units is a microdose of semaglutide?

Units depend on the concentration printed on the compounding pharmacy's label, so there is no single answer. Follow your prescriber's instructions and the pharmacy label, and ask the pharmacist to confirm the dose. FDA has warned of dosing errors with compounded injectable semaglutide, some needing hospital care [11].

Can I stay on a low dose of Wegovy?

The Wegovy label lists 1.7 mg and 2.4 mg weekly as the maintenance doses for adult weight reduction, so staying on 0.25 mg or 0.5 mg is outside the label. If a step is not tolerated, the label lets the prescriber delay escalation by 4 weeks [1].

Is microdosing semaglutide safe?

The label warnings apply at any dose, including the boxed warning on thyroid C-cell tumors and the pancreatitis warning [1]. Compounded semaglutide is not FDA-approved, and FDA had received 990 adverse event reports for it as of May 31, 2026 [11].

What happens when you stop microdosing semaglutide?

No study has tracked people who stop a microdose. After stopping full-dose semaglutide in the STEP 1 extension, people regained 11.6 of the 17.3 percentage points they had lost within one year [14].
