Zepbound and Mounjaro are the same drug, tirzepatide, under two FDA labels; insurance coverage follows the label, so Zepbound requests are judged on weight or sleep apnea criteria and Mounjaro requests on type 2 diabetes [1] [2].

Two labels, two sets of coverage rules
| Zepbound | Mounjaro | Source | |
|---|---|---|---|
| Approved uses | Weight management in adults with obesity, or overweight with a weight-related condition; moderate to severe sleep apnea with obesity | Blood sugar control in type 2 diabetes (age 10+); heart risk reduction in high-risk adults with type 2 diabetes | [1] [2] |
| BMI in the FDA approval | 30 or more, or 27 or more with a weight-related condition | Not a weight drug | [7] |
| Savings card with coverage | $25; up to 13 fills a year | $25; up to 13 fills a year | [3] [4] |
| Savings card without coverage | $499 single-dose pen; KwikPen $299 to $449 | $499 | [3] [4] |
| Medicare GLP-1 Bridge | KwikPen only, $50 | Not covered | [6] |
The four questions your plan asks
- Did your employer or plan include weight-loss drugs? Lilly cites survey data that about half of Americans with employer coverage lack coverage for obesity medicines [8]. If yours excludes them, a sleep apnea or diabetes diagnosis may still open a door.
- Is the drug on the formulary, and is another preferred? North Carolina Medicaid, for instance, covers Zepbound only after a Wegovy trial [9].
- Do you meet prior authorization criteria? Published examples mirror the label: TennCare's Aug 1, 2025 notice covers Zepbound with prior authorization for a BMI of 30 or more, or 27 or more with a related condition [10].
- Does the diagnosis match the brand? A Mounjaro request without type 2 diabetes is off-label, and Lilly's Mounjaro card requires an FDA-approved use [4].
Sleep apnea: the other route to Zepbound coverage
The FDA approved Zepbound for moderate to severe obstructive sleep apnea in adults with obesity on Dec 20, 2024 [11]. In the label's trials, apnea events fell by about half or more after a year [12]. Some plans that dropped weight-only coverage still cover sleep apnea: MassHealth covers adults with a BMI over 30 and moderate to severe sleep apnea [13], and Utah Medicaid keeps Zepbound for sleep apnea with prior authorization [14]. On Medicare, people with sleep apnea are excluded from the Bridge, and Medicare says their Part D plan might cover a GLP-1 [15]. Ask your plan whether the request needs a sleep study result.

Getting it covered, including online
- Pull up your plan's formulary and look for Zepbound and Mounjaro, with any prior authorization or step therapy flags.
- Ask the plan what documents the request needs: BMI, related conditions, a sleep study, or diabetes labs.
- Have your prescriber submit it. Online programs that work with insurance can do this; LifeMD includes insurance help in its program fee [17], and Form Health takes insurance including Medicare [18].
- When approved, activate Lilly's savings card if you have commercial insurance [3].
If you are denied
- Appeal with your prescriber. Check the denial letter for the reason and any appeal deadline; we could not confirm a standard deadline, so ask your plan and act quickly. Add any missing BMI, diagnosis or sleep study records.
- Ask about the preferred drug. If the plan prefers Wegovy, see Wegovy insurance.
- Use the uncovered-plan card. With commercial insurance that does not cover Zepbound, the KwikPen costs $299 to $449 with Lilly's card [3].
- Pay cash. LillyDirect sells Zepbound at $299 to $449 when refilled within 45 days [5]. Compounded tirzepatide is cheaper but not FDA-approved [19]; read tirzepatide cost first.
Medicaid and tirzepatide
In January 2026, 13 state Medicaid programs covered GLP-1s for obesity [20]. Tennessee covered Zepbound with prior authorization under its Aug 1, 2025 notice [10], North Carolina covered it after a Wegovy trial when it reinstated coverage in December 2025 [9], and Michigan limits coverage to morbid obesity after other treatments fail [21]. Medi-Cal ended adult weight-loss coverage on Jan 1, 2026, but covers other uses when medically necessary [22]. We could not confirm changes after April 2026 outside Massachusetts and Utah. The full state table is on our Medicare and Medicaid page.
Tirzepatide safety, covered or not
Zepbound and Mounjaro carry a boxed warning for thyroid C-cell tumors seen in rats; do not use them with a personal or family history of medullary thyroid carcinoma or MEN 2 [12]. If stomach pain is severe and does not let up, or you notice signs of an allergic reaction, contact a clinician right away. Taking insulin or a sulfonylurea at the same time raises the chance of low blood sugar. Oral birth control may work less well for 4 weeks after starting and after each dose increase [2]. Stop Zepbound when pregnancy is recognized [12]. For an emergency, call 911.

Questions people ask
Does UnitedHealthcare, Anthem or Blue Cross cover Zepbound?
Coverage depends on your employer's plan, not the carrier name. One insurer runs many plans, and each employer chooses whether obesity drugs are included. To check: call the pharmacy number on your card, ask whether Zepbound is on your formulary for weight management or sleep apnea, ask what prior authorization needs, and ask your copay. If covered, Lilly's savings card can bring Zepbound to $25 [3].
Does Aetna, Cigna or BCBS cover Mounjaro?
Coverage depends on your employer's plan, not the carrier name. Ask your plan whether Mounjaro is on the formulary for type 2 diabetes and what prior authorization needs. Mounjaro is approved only for type 2 diabetes uses [2]. If covered, Lilly's card can bring the cost to $25 for a 1-, 2- or 3-month fill [4].
How do I get Zepbound covered by insurance?
Confirm Zepbound is on your plan's formulary, find out the prior authorization criteria, and have your prescriber submit records showing you meet them, such as a BMI of 30 or more, or 27 or more with a weight-related condition, matching the FDA approval [7], or moderate to severe sleep apnea with obesity [1]. Then use Lilly's savings card to bring the copay to as little as $25 [3].
How do I get Mounjaro covered without diabetes?
Will insurance cover Zepbound or Mounjaro for sleep apnea?
Zepbound, yes, if your plan covers it for that use: the FDA approved it for moderate to severe sleep apnea in adults with obesity on Dec 20, 2024 [11]. Mounjaro is not approved for sleep apnea [2]. Some state Medicaid programs, such as MassHealth, still cover sleep apnea use after ending weight-only coverage [13].
