Mounjaro is Eli Lilly's brand of tirzepatide, a once-weekly GIP and GLP-1 receptor agonist injection approved for type 2 diabetes in adults and children 10 and older.
What Mounjaro is
Mounjaro is tirzepatide, made by Eli Lilly. Tirzepatide is "a GIP receptor and GLP-1 receptor agonist" [8], so Mounjaro is a GLP-1 drug that also works on a second gut hormone receptor, GIP. It is not semaglutide (the drug in Ozempic) and it is not insulin. Is Mounjaro a peptide? The label describes tirzepatide as built on the sequence of the GIP hormone with a fatty acid chain attached, so yes, a modified peptide [8]. Full background: tirzepatide.
What Mounjaro is used for
- Improving blood sugar, with diet and exercise, in adults and children 10 and older with type 2 diabetes [3].
- Lowering the risk of cardiovascular death, heart attack and stroke in adults with type 2 diabetes at high risk [3].
Mounjaro is not approved for weight loss [6]. The same molecule is approved for weight as Zepbound. See Mounjaro vs Zepbound and Ozempic vs Mounjaro.
What Mounjaro does in the body
Tirzepatide binds and activates both the GIP and GLP-1 receptors, lowers calorie intake (likely through appetite), and slows stomach emptying, with the largest delay after the first dose [8]. Levels peak about a day after a shot, and the half-life is about 5 to 6 days [8]. How does it make you feel? The most common effects in diabetes trials were stomach-related: nausea, diarrhea and less appetite [3]. Class overview: how GLP-1 medicines work.
Doses and how to take it
Start at 2.5 mg once a week. If more blood sugar control is needed, the dose rises by 2.5 mg after at least 4 weeks on the current dose, up to 15 mg for adults and 10 mg for children. The label does not list a separate set of maintenance doses [3]. Inject in the abdomen or thigh, or the back of the upper arm if someone else gives it; rotate sites; any time of day, with or without food [3].
Can you take Mounjaro a day early? You can change the dosing day if doses stay at least 3 days (72 hours) apart. A missed dose can be taken within 4 days; after that, skip it [3]. The KwikPen is not recommended for self-injection by people with vision problems or by children, and it must never be shared, even with a new needle [1]. Is Mounjaro safe?
| Side effect | Placebo | 5 mg | 10 mg | 15 mg |
|---|---|---|---|---|
| Nausea | 4 | 12 | 15 | 18 |
| Diarrhea | 9 | 12 | 13 | 17 |
| Decreased appetite | 1 | 5 | 10 | 11 |
| Vomiting | 2 | 5 | 5 | 9 |
| Constipation | 1 | 6 | 6 | 7 |
Low blood sugar depends on the other drugs you take. Used alone, Mounjaro trials showed 0% with blood sugar below 54 mg/dL vs 1% on placebo. With basal insulin it was 14% to 19% (13% placebo), and with a sulfonylurea 9.9% to 13.8% [3]. Tirzepatide carries a boxed warning for thyroid C-cell tumors in rats and is not for anyone with a personal or family history of medullary thyroid carcinoma or MEN 2 [8].
Birth control pills may work less well: switch to a non-oral method or add a barrier method for 4 weeks after starting and after each dose increase [3]. In pregnancy, the label says to use it only if the benefit justifies the risk to the fetus [3]. Mixing it with Contrave, Jardiance or other drugs is a question for your pharmacist; start with Mounjaro interactions.

Results
Mounjaro's label is about blood sugar and heart risk, not weight. For weight results of tirzepatide in adults with type 2 diabetes, the Zepbound label reports average losses of 12.8% on 10 mg and 14.7% on 15 mg at 72 weeks, vs 3.2% on placebo [8]. More: Mounjaro results and stopping Mounjaro.
