How we check

Does Blue Cross Blue Shield cover Zepbound, Wegovy, Ozempic or Mounjaro?

It depends on which Blue company runs your plan and what your employer bought: Blue Cross Blue Shield is an association of 33 independent, locally operated companies (the same BCBSA page also says 35), and each one writes its own drug rules [1]. As of Oct 18, 2026, FEP Blue covers Wegovy and Zepbound for weight loss with prior approval [2] [3], while BCBS of Vermont ended standard weight-loss coverage on Jan 1, 2026 and BCBS of Massachusetts began ending it that day, as plans renew during 2026 [4] [5]. Ozempic and Mounjaro are type 2 diabetes drugs [6] [7], and both of those plans kept covering GLP-1s for diabetes [4] [5].

Medical review: pending. A licensed clinician has not yet reviewed this page.

Blue Cross Blue Shield is a national association that owns the Blue names and licenses them to independent, locally run companies, and each company sets its own drug coverage [1].

Why there is no single Blue Cross answer

The Blue Cross Blue Shield Association owns and manages the Blue names and grants licenses to independent companies [1]. The Blue logo on your card is the brand. The company name next to it, such as Anthem, Highmark, Florida Blue or Premera, tells you whose drug policy applies.

The second layer is your employer. Several Blue companies say plainly that weight-loss drugs are left out unless the plan buys them: Premera says these drugs "are excluded under many benefit plans" [9], Wellmark treats them as a standard exclusion on most plans [10], and Blue Cross and Blue Shield of Texas tells members "Most plans don't cover GLP-1s for weight loss" [11]. Self-funded employers often get a choice that fully insured groups do not. Blue Cross NC's Vida GLP-1 program is for self-funded (ASO) plans only [12], and when BCBS of Michigan ended coverage for fully insured large groups, self-funded groups could keep their own criteria [13]. Compare this with other carriers on our GLP-1 insurance coverage tracker.

One class of drugs, four different coverage questions

A plan can refuse Wegovy for weight loss and still pay for the same drug when it is prescribed for heart risk, or pay for Ozempic for diabetes. Ask about the use your prescriber is writing for.

How Blue plans treat each use, checked Oct 18, 2026
UseDrug approved for itWhat the Blue plans we checked doSource
Weight lossWegovy, Zepbound (adults with obesity, or overweight with a weight-related condition)Covered only if your plan includes the benefit at most plans checked. FEP Blue covers it with prior approval. Ended at BCBS of Massachusetts and Vermont on Jan 1, 2026[29] [30] [2] [4] [5]
Heart riskWegovy, approved Mar 8, 2024 to cut the risk of heart attack, stroke and cardiovascular death in adults with heart disease and obesity or overweightPremera: BMI 27+ with a prior heart attack, stroke or symptomatic PAD. Vermont's FAQ keeps Wegovy, with prior approval, for adults with diagnosed cardiovascular disease and obesity. FEP's Wegovy form lists the heart indication[31] [9] [5] [14]
Sleep apneaZepbound, for moderate to severe obstructive sleep apnea in adults with obesityPremera: BMI 30+ and an AHI of 15+ on a sleep study within 12 months. FEP lists sleep apnea on its Zepbound form, but the policy has no separate sleep apnea criteria. Highmark revised its sleep apnea criteria effective Oct 2, 2025[30] [9] [8] [3] [20]
Type 2 diabetesOzempic and MounjaroBCBS of Massachusetts covers GLP-1s only for type 2 diabetes; Vermont still covers Ozempic, Mounjaro and Trulicity with prior approval. Florida Blue's January 2025 notice required, from Apr 1, 2025, prior authorization with a type 2 diabetes diagnosis and a tried-and-failed metformin, metformin combination or insulin[6] [7] [4] [5] [18]
Premera and Wellmark list heart and sleep apnea criteria, but whether those uses get past a plan's weight-loss exclusion was not confirmed [9] [10]. Drug-specific detail: Wegovy insurance, Zepbound insurance, Ozempic insurance.

FEP Blue for federal employees

The Federal Employee Program covers roughly 5.6 million members [1]. FEP Blue says: "We cover weight-loss drug prescriptions to treat obesity for Service Benefit Plan members who meet eligibility criteria," and members must get prior approval [2].

The 2026 rules

  • Who qualifies: age 18 or older, BMI 30 or more, or 27 or more with established heart disease or a weight-related condition, and taking part in a comprehensive weight management program such as Teladoc [3] [14].
  • Standard and Basic options: "The preferred medications are Saxenda and Wegovy." Zepbound is non-preferred: the form asks whether you tried and failed both [8], and the policy also requires at least two oral weight-loss drugs first [3].
  • Blue Focus: Zepbound is non-formulary, and the preferred drug is generic liraglutide [8].
  • Limits and renewal: Zepbound is limited to 12 single-dose pens per 84 days [3]. Wegovy renewal needs at least 5% weight loss, or keeping that 5% off [14].
  • Other uses: the Wegovy form lists heart risk reduction and noncirrhotic MASH [14]. The Zepbound form lists moderate to severe sleep apnea with obesity, but the policy's criteria cover weight management only [8] [3].

2027: what OPM told carriers

In Carrier Letter 2026-07, dated Mar 31, 2026, the Office of Personnel Management said that for plan year 2027 it "continues to require" FEHB carriers to offer on their formularies "at least one anti-obesity medication (AOM) from the GLP-1 class for weight loss" plus at least two oral anti-obesity drugs, and that these drugs "must only be covered when used concurrently with" intensive behavioral therapy. App-only or AI-only vendors do not count as that therapy [32]. This is OPM's instruction, not FEP Blue's final 2027 benefit. FEP Blue's 2027 "What's new" page names no GLP-1 change; it announces Accredo as its specialty pharmacy from January 2027 [33]. FEP Blue's 2027 tiers and criteria for these drugs were not confirmed as of Oct 18, 2026, and neither were its current Ozempic and Mounjaro criteria.

What Blue prior authorizations ask for

  • BMI: five of the published Blue policies we read use 30 or more, or 27 or more with a weight-related condition (FEP, Regence, Premera, Wellmark, Blue Cross NC) [3] [26] [9] [10] [12]. Brokers relaying a Horizon notice report a baseline BMI of 35 with a comorbidity from Jan 1, 2026 for fully insured Public Sector and some ASO plans [23] [24]. Blue Shield of California's exception is for Class III obesity [27].
  • A lifestyle program: FEP asks for a comprehensive weight management program [14]; Premera wants at least 3 months of behavioral change and diet [9]; Wellmark asks for a program that encourages behavior change [10]; Blue Cross NC requires enrollment in Vida [12].
  • A preferred drug first: FEP puts Wegovy and Saxenda ahead of Zepbound [8]. Highmark's January 2026 update adds a Zepbound trial before Foundayo (orforglipron) under its Standard policy [21]. Regence states "No preference is given to any one medication" [26].
  • Proof it is working: FEP's Wegovy renewal needs 5% weight loss [14].

Dated changes at Blue plans, newest first

  1. Oct 1, 2026: Anthem's California Essential list, current as of this date, shows the Zepbound KwikPen at Tier 2 with prior authorization, and we found no Wegovy in its anti-obesity section [16].
  2. Sept 18, 2026: FEP's Wegovy prior authorization form revised [14].
  3. Sept 1, 2026: Premera policy 5.01.621 and Wellmark policy 05.04.33 take effect, both naming Foundayo [9] [10].
  4. Aug 15, 2026: Regence policy dru787.6 takes effect [26].
  5. May 20, 2026: HCSC tells self-funded groups the Zepbound KwikPen is excluded and Wegovy HD is added [17].
  6. Mar 31, 2026: OPM's 2027 call letter requires FEHB carriers to cover a GLP-1 for weight loss with behavioral therapy [32].
  7. Jan 1, 2026: BCBS of Vermont ends standard weight-loss coverage and BCBS of Massachusetts starts ending it as plans renew; Excellus drops Wegovy, Zepbound and Saxenda from its list for certain plans, per Rochester First; Horizon's BMI 35 policy starts, per brokers; FEP's Zepbound policy 5.99.031 takes effect [4] [5] [28] [23] [3].
  8. Aug 6, 2025: Highmark's enhanced anti-obesity policy revised for Wegovy, Zepbound and Saxenda [20].
  9. July 1, 2025: CareFirst drops Zepbound from five formularies, keeping Wegovy and Saxenda [25].
  10. Apr 1, 2025: Florida Blue starts requiring prior authorization for GLP-1s on commercial plans [18].
  11. Jan 1, 2025: BCBS of Michigan ends coverage for fully insured large groups; Independence stops covering weight-loss-only prescriptions, per WHYY; Blue Cross NC starts Vida for ASO plans; Blue Shield of California's exclusion begins on certain plans [13] [22] [12] [27].

Finding your own Blue plan's rule

  1. Read the company name on your card. That is the Blue company whose policy applies, not the association.
  2. Call the pharmacy or member services number on the card. Ask whether Wegovy, Zepbound or Foundayo is on your formulary for weight management, and whether your plan design excludes weight-loss drugs.
  3. Ask for the prior authorization criteria document by name, and which drug is preferred.
  4. Ask your HR team whether the plan is self-funded and whether a weight-loss rider or opt-in exists. BCBS of Massachusetts keeps coverage only through an employer rider [4].
  5. If the prescription is for heart risk, sleep apnea or diabetes, ask about that use separately.

When your Blue plan says no

  • Appeal with your prescriber, adding any missing BMI, diagnosis or sleep study records. One exception: BCBS of Massachusetts says weight-loss coverage "cannot be appealed" [4].
  • Use Lilly's card if you have commercial insurance that does not cover Zepbound: the KwikPen costs as low as $299 to $449 a month depending on dose, with the $449 doses (7.5 mg and up) needing a refill within 45 days [34].
  • Pay cash for Zepbound: $299 for 2.5 mg, $399 for 5 mg and $449 for 7.5 mg and up per 28-day month on LillyDirect, the $449 price when each refill comes within 45 days, checked Oct 17, 2026 [35]. See Zepbound cost.
  • Pay cash for Wegovy: $349 a month for the pen through NovoCare, or $199 for a new self-pay patient's first two fills of the two lowest doses through Dec 31, 2026; the Wegovy pill is $149 to $299, checked Oct 17, 2026 [36]. See Wegovy cost and the GLP-1 price tracker.
  • Age 65 or older: Medicare's GLP-1 Bridge charges $50 a month for eligible Part D members [37]. Details on our Medicare page.

Safety, whatever your plan pays

Wegovy, Ozempic, Zepbound and Mounjaro carry a boxed warning for thyroid C-cell tumors seen in rodents; do not use them with a personal or family history of medullary thyroid carcinoma or MEN 2 [29] [30]. Get care right away for severe stomach pain that will not go away or for signs of an allergic reaction. Taking insulin or a sulfonylurea at the same time raises the chance of low blood sugar. Wegovy's label says to stop at least 2 months before a planned pregnancy [29], and Zepbound's says to stop when pregnancy is recognized [30]. For an emergency, call 911.

Questions people ask

Does Blue Cross cover Zepbound?

It depends on the Blue company and the plan. FEP Blue covers Zepbound for weight loss with prior approval on Standard and Basic, but only after Wegovy, Saxenda and two oral weight-loss drugs, and not on Blue Focus [8] [3]. CareFirst dropped Zepbound from five formularies on July 1, 2025 [25], and BCBS of Massachusetts and Vermont stopped covering it for weight loss starting Jan 1, 2026, Massachusetts as plans renew [4] [5]. Call the number on your card and ask whether Zepbound is on your formulary for weight management or sleep apnea.

Does Blue Cross Blue Shield cover Wegovy for weight loss?

Only if your Blue company and plan include weight-loss drugs. FEP Blue covers Wegovy with prior approval and prefers it on Standard and Basic [8]. Premera, Wellmark and Regence cover it only where the plan includes the benefit [9] [10] [26]. Florida Blue excludes weight-loss drugs on commercial plans [18], and BCBS of Massachusetts began ending coverage on Jan 1, 2026, as plans renew [4].

Does Blue Cross Blue Shield cover Ozempic?

Ozempic is approved for type 2 diabetes, not weight loss [6]. Blue plans that cut weight-loss coverage kept diabetes coverage: BCBS of Vermont still covers Ozempic, Mounjaro and Trulicity with prior approval [5], and BCBS of Massachusetts covers GLP-1s for type 2 diabetes [4]. Florida Blue's January 2025 notice required a type 2 diabetes diagnosis and a tried-and-failed metformin, metformin combination or insulin from Apr 1, 2025 [18].

Does BCBS cover Ozempic for weight loss?

Ozempic is not FDA-approved for weight loss [6], so a weight-loss request is usually judged under the plan's weight-loss rules, which cover Wegovy or Zepbound instead. BCBS of Massachusetts says GLP-1s "will only be covered for type 2 diabetes" [4]. If your plan covers weight-loss drugs, ask about Wegovy, which is the same drug, semaglutide, under a weight-management label [29].

Does BCBS cover Mounjaro?

Mounjaro is approved for type 2 diabetes [7], and Blue plans that dropped weight-loss drugs kept diabetes coverage: BCBS of Vermont covers Mounjaro with prior approval [5], and BCBS of Michigan said it would keep covering GLP-1s for type 2 diabetes [38]. For weight loss, the matching drug is Zepbound, covered only if your plan includes it.

Does FEP Blue cover Zepbound?

Yes, with prior approval, on FEP Blue Standard and Basic, where Zepbound is non-preferred: you must have tried Saxenda and Wegovy and at least two oral weight-loss drugs [8] [3]. On FEP Blue Focus, Zepbound is non-formulary and generic liraglutide is preferred [8]. The limit is 12 single-dose pens per 84 days [3].

Does FEP Blue cover Wegovy?

Yes, with prior approval. Wegovy and Saxenda are FEP's preferred weight-loss drugs on Standard and Basic [8]. You need a BMI of 30 or more, or 27 or more with a weight-related condition or heart disease, and a comprehensive weight management program; renewal needs at least 5% weight loss [14]. For 2027, OPM requires FEHB carriers to cover at least one GLP-1 for weight loss with behavioral therapy [32].

Does BCBS of Texas or Illinois cover Wegovy for weight loss?

Most plans do not: Blue Cross and Blue Shield of Texas tells members "Most plans don't cover GLP-1s for weight loss" [11]. HCSC, which runs the Texas, Illinois, Oklahoma, New Mexico and Montana Blue plans, lets self-funded groups elect coverage of Wegovy, Wegovy HD and the Zepbound auto-injector, and told those groups in a May 20, 2026 notice that the Zepbound KwikPen is excluded [17].

Does Highmark cover Wegovy?

Highmark revised its Anti-Obesity (Enhanced) policy for Wegovy, Zepbound and Saxenda on commercial and Healthcare Reform (ACA) plans, effective Aug 6, 2025 [20]. A January 2026 update adds a Zepbound trial before Foundayo (orforglipron) under its Standard policy [21]. A reported BMI 40 rule could not be confirmed from Highmark's own policy as of Oct 18, 2026. Ask Highmark which policy your plan uses.

Does CareFirst cover Wegovy?

CareFirst kept Wegovy and Saxenda as alternatives when it dropped Zepbound from its F2, F3, F3 Choice, F2 CHART and F4 CHART formularies on July 1, 2025 [25]. Whether your CareFirst plan covers weight-loss drugs at all depends on its design, which we could not confirm as of Oct 18, 2026.
