Medicare: what the GLP-1 Bridge covers
| Drug | Under the GLP-1 Bridge (weight management) | Through your regular Part D plan | Source |
|---|---|---|---|
| Zepbound KwikPen | Covered, $50 a month | Might be covered for moderate to severe sleep apnea | [1] [2] |
| Zepbound vials and single-dose pens | Not covered | Might be covered for sleep apnea, per plan | [1] [2] |
| Wegovy pen or pill | Covered, $50 a month | Ask your plan about heart-risk or fatty liver uses | [1] [2] |
| Foundayo | Covered, $50 a month | Not applicable | [1] |
| Ozempic | Not covered | Might be covered for type 2 diabetes | [1] [2] |
| Mounjaro | Not covered | Might be covered for type 2 diabetes | [1] [2] |
Who qualifies for the Bridge
- You have Medicare Part D drug coverage and are 18 or older [2].
- Your BMI is 35 or higher; or 30 or higher with heart failure with preserved ejection fraction, uncontrolled high blood pressure, or chronic kidney disease stage 3a or worse; or 27 or higher with prediabetes, a past heart attack or stroke, or symptomatic peripheral artery disease [2].
- You do not have type 2 diabetes, moderate to severe sleep apnea or fatty liver disease (MASH); those patients use their regular Part D plan [2].
- Your prescriber confirms you are in a diet and exercise lifestyle program [2].
How the Bridge works in practice
- Prior authorization is processed centrally by Humana, with a decision within 72 hours. Approval lasts through Dec 31, 2027 unless you switch drugs [1].
- Fills are 28- or 30-day supplies only, with no 90-day fills, and pharmacies do not have to opt in [1].
- The $50 copay does not count toward your Part D deductible or out-of-pocket cap, is not reduced by Extra Help, cannot be paid through the Medicare Prescription Payment Plan, and coupons cannot be applied [2].
- Pen needles are not covered [1].
- The Bridge was first set for six months and was extended to 18 months in April 2026 [6]. NCOA reports makers supply Bridge drugs at a net price of about $245 a month [7].
What about BALANCE?
CMS planned a second program, BALANCE, to bring negotiated GLP-1 coverage into Part D in January 2027. AARP reports the Medicare part was postponed indefinitely in April 2026, while the Medicaid part launched in May 2026 and runs through 2031 [6].
Medicare without the Bridge
If you do not qualify, maker savings cards are off the table: Lilly and Novo exclude Medicare members from their $25 offers [4] [5]. You can still pay self-pay prices, such as $299 to $449 for Zepbound on LillyDirect [8] or $349 for Wegovy pens on NovoCare [5]. Lilly said eligible Part D members may get Foundayo for $50 a month from July 1, 2026 [9]. Compare every route in the GLP-1 Price Tracker.
Two approvals matter here. The FDA approved Wegovy to reduce heart attack, stroke and cardiovascular death risk in adults with heart disease and obesity or overweight on March 8, 2024 [10], and Zepbound for moderate to severe sleep apnea with obesity on Dec 20, 2024 [11]. Ask your Part D plan whether it covers the drug for those uses.
Medicaid coverage by state
As of January 2026, 13 state Medicaid programs covered GLP-1s for obesity, down from 16 in October 2025. California, New Hampshire, Pennsylvania and South Carolina ended coverage, and North Carolina dropped it and then brought it back [3]. Medicaid may still cover GLP-1s for type 2 diabetes or other approved uses where obesity coverage ended, as South Carolina and California do [12] [13]. We could not confirm any state's Medicaid status after April 2026 except Massachusetts and Utah, so treat the covered states below as last known status.
| State | Status | Detail | Source |
|---|---|---|---|
| Tennessee | Covered as of April 2026 | TennCare covers Wegovy and Zepbound with prior authorization for BMI of 30 or more, or 27 or more with a related condition, since Aug 1, 2025 | [14] |
| North Carolina | Covered as of Dec 2025 | Dropped Oct 1, 2025, reinstated Dec 12, 2025. Wegovy preferred; Zepbound needs a Wegovy trial first. | [15] |
| Minnesota, Missouri, Virginia | Covered as of April 2026 | Named among covering states in April 2026 | [16] |
| Michigan | Limited as of Jan 2026 | Since Jan 1, 2026, only for members with morbid obesity who failed other treatments, to avoid bariatric surgery | [17] |
| Utah | Not covered for weight alone | Pilot ended with reauthorizations through June 30, 2026. Wegovy (heart, MASH) and Zepbound (sleep apnea) still covered with prior authorization. | [18] |
| Massachusetts | Not covered for weight alone | Ended July 3, 2026. Still covered for BMI over 27 with heart disease, BMI over 30 with sleep apnea, MASH, and under 21. | [19] |
| California | Not covered for adult weight loss | Ended Jan 1, 2026; members under 21 still covered | [13] |
| Pennsylvania | Not covered for weight | Ended Jan 1, 2026; other GLP-1 uses need prior authorization | [20] |
| South Carolina | Not covered for weight | Ended Jan 1, 2026; diabetes coverage continues | [12] |
| Arizona, Colorado, DC, Florida, Georgia, Illinois, Indiana, Maryland, Nevada, New Jersey, New York, Ohio, Oregon, Washington | Not covered for obesity | Not among the 13 programs KFF counted as covering obesity GLP-1s in January 2026 | [3] |
| Louisiana, New Mexico, Texas | Not covered for obesity | KFF lists these as covering some weight-loss drugs but no GLP-1s for obesity | [3] |
Commercial insurance
For job-based and individual plans, coverage depends on whether your employer or plan added obesity drugs and on its prior authorization rules. Lilly, citing an IFEBP survey, says about half of Americans with employer coverage lack coverage for obesity medicines [9]. If your plan covers the drug, maker savings cards can bring the cost to $25 [4] [5]. Step-by-step guides: Wegovy, Zepbound and Mounjaro, Ozempic.
Safety does not depend on who pays
GLP-1 labels carry a boxed warning for thyroid C-cell tumors seen in rodents and warn about pancreatitis, gallbladder disease and low blood sugar with insulin or a sulfonylurea [21] [22]. Call your prescriber about stomach pain that is severe and lasting, or about rash, swelling or trouble breathing; for an emergency, call 911.

Questions people ask
Does Medicare cover Zepbound?
For weight management, Medicare covers only the Zepbound KwikPen, at a $50 monthly copay under the GLP-1 Bridge from July 1, 2026 through Dec 31, 2027, for Part D members who meet the criteria. Zepbound vials and single-dose pens are excluded [1]. People with sleep apnea are not eligible for the Bridge, but their Part D plan might cover a GLP-1 [2].
Does Medicare cover Wegovy in 2026?
Does Medicare cover Ozempic?
Does Medicare cover Mounjaro?
Who qualifies for the Medicare GLP-1 Bridge?
Part D members 18 or older with a BMI of 35 or more; or 30 or more with HFpEF heart failure, uncontrolled high blood pressure or stage 3a or worse kidney disease; or 27 or more with prediabetes, a past heart attack or stroke, or symptomatic peripheral artery disease. People with type 2 diabetes, moderate to severe sleep apnea or MASH are excluded [2].
Is Wegovy covered by Medicare Part B?
The coverage Medicare describes for Wegovy runs through Part D: the GLP-1 Bridge requires Part D coverage and charges $50 a month [2].
Does Medicaid cover Ozempic or Wegovy?
It depends on the state. In January 2026, 13 state Medicaid programs covered GLP-1s for obesity [3]. North Carolina reinstated coverage in December 2025 with Wegovy as the preferred drug [15], while California, Pennsylvania and South Carolina ended obesity coverage on Jan 1, 2026 [13] [20] [12]. We could not confirm changes after April 2026 outside Massachusetts and Utah, so ask your state plan.
Does Medicaid cover tirzepatide or Mounjaro for weight loss?
Only in some states, and we could not confirm any changes after April 2026 outside Massachusetts and Utah. TennCare's Aug 1, 2025 notice covers Zepbound with prior authorization for a BMI of 30 or more, or 27 or more with a related condition [14]. North Carolina's December 2025 reinstatement covers Zepbound after a Wegovy trial [15]. Many states do not cover any GLP-1 for obesity [3].
