How we check

Does UnitedHealthcare cover Wegovy, Zepbound, Ozempic or Mounjaro?

On a standard UnitedHealthcare commercial plan, Wegovy and Zepbound prescribed for weight loss are a benefit exclusion, but the same drug list allows Wegovy injection coverage for heart risk and MASH, and Zepbound coverage for sleep apnea, under separate criteria, according to UHC's drug list for Oct 1, 2026 [1] [2]. Employers can add weight-loss coverage, and those plans use UHC's prior authorization program 2026 P 1114-23, effective Oct 1, 2026 [2]. Ozempic and Mounjaro are covered for type 2 diabetes with prior authorization [3].

UnitedHealthcare is UnitedHealth Group's health insurer; its commercial pharmacy benefit is run by Optum Rx, and whether a GLP-1 is covered depends on the drug, the diagnosis and whether the employer bought weight-loss coverage [1] [2].

UnitedHealthcare coverage by drug and use

UHC judges a GLP-1 request by the reason it is prescribed, so the same drug can be excluded for one use and covered for another. This table reads across the four uses that matter for these drugs. It describes UHC commercial plans (fully insured and self-funded employer plans). Your plan document always controls.

Standard plans means UHC commercial plans that have not bought the weight-loss benefit. Buy-up plans are employer plans that elected it. Separate "nonformulary Wegovy" and "nonformulary Zepbound" criteria govern the heart, MASH and sleep apnea uses on standard plans; we could not locate those documents as of Oct 18, 2026 [2]. Also see Wegovy insurance and Zepbound insurance.

Standard plans and buy-up plans

UHC runs its weight-loss program as an add-on. Its own wording: "This is an optional program that is put in place for clients or businesses" that elect to cover weight-loss drugs [2]. If your employer did not elect it, the drug list marks Wegovy as "Benefit exclusion. Injectable coverage for MASH and CV only." and Zepbound as "Benefit exclusion. Coverage for OSA only." [1]

Becker's reported that UHC's Total Weight Support offering, which pairs a lifestyle program (Real Appeal Rx or WeightWatchers for Business) with prior-authorization coverage of the drugs, is sold only to self-insured employers [11]. That limit comes from the news report, not a UHC document. Separately, on April 30, 2026, Optum Rx announced Weight Engage, a support program with Calibrate, Omada Health and Virta Health that employers and health plans can configure; the announcement gives no drug coverage terms [10].

As of our Oct 18, 2026 check of the UHC policy documents cited on this page [1] [2], we could not confirm how UHC individual (ACA marketplace) plans treat weight-loss GLP-1s.

What UHC's prior authorization asks for

For plans that cover weight loss, program 2026 P 1114-23 (effective Oct 1, 2026) sets these rules [2]:

  • Age: 12 or older for Wegovy injection, Saxenda, Qsymia and Xenical; over 16 for the others [2].
  • BMI: 30 or higher (above the 95th percentile for children), or 27 or higher with a weight-related condition such as high cholesterol, high blood pressure, type 2 diabetes or sleep apnea [2].
  • Lifestyle: the drug must be used alongside lifestyle changes [2].
  • First approval: 5 months for Wegovy, 6 months for Zepbound and Foundayo, 4 months for Saxenda [2].
  • Renewal: at least 5% weight loss from baseline (4% for Saxenda) and continued lifestyle changes, approved for 12 months. One extra 6-month approval is allowed if you have not yet reached the maintenance dose [2].
  • Heart risk: the Wegovy heart criteria require established cardiovascular disease, meaning a prior heart attack, stroke or peripheral artery disease [2].
  • No preferred drug: the program document states no preferred product among Wegovy, Wegovy HD, Zepbound, Foundayo and Saxenda [2].

Ozempic and Mounjaro: the diabetes policy

UHC covers Ozempic, Mounjaro, Rybelsus, Trulicity and Victoza for type 2 diabetes under program 2026 P 1332-8, effective July 1, 2026 [3]. The request needs records confirming the diagnosis, such as an A1C of 6.5% or higher or a fasting glucose of 126 or higher. No metformin trial is required; the step therapy language was removed in April 2024. UHC may approve from claims history or diagnosis codes, and "Authorization will be issued for 12 months." [3]

The same policy says "Medications used for the purposes of weight loss are typically excluded from benefit coverage" [3]. Asking for Ozempic or Mounjaro without diabetes is unlikely to pass. For the weight-loss versions of the same molecules, see Ozempic vs Wegovy and Mounjaro vs Zepbound.

Optum Rx formulary tiers, 2026 and 2027

Optum Rx, UnitedHealth Group's pharmacy benefit manager, also publishes standard formularies (Premium and Select) that employers and plans contracting with it can adopt. A tier listing is not the same as weight-loss coverage: the "++" flag means coverage depends on the plan's benefit design [4]. The 2027 booklets, effective Jan 1, 2027, are already posted [7] [8].

PA = prior authorization. QL = quantity limit. ++ = covered only if the plan's benefit design includes it. Wegovy HD, added in Optum's April 2026 Pharmacy Passages update, is not printed in the July 1, 2026 booklets, so a drug missing from a booklet may still be on the formulary [4] [5] [6]. Ozempic and Mounjaro are listed as diabetes drugs and carry no ++ flag [4].

The clear 2027 change is Saxenda: the brand moves to excluded on Premium, with generic liraglutide at Tier 1 [7]. Foundayo appears at Tier 2 on both 2027 lists [7] [8]. In January 2026, Optum said immediate changes to commercial GLP-1 pricing were "unlikely given existing contractual agreements" after the federal pricing actions [9].

What changed at UnitedHealthcare, 2025 to 2026

  • Jan 1, 2027 (published): Optum Rx 2027 lists exclude brand Saxenda on Premium and add Foundayo at Tier 2 [7] [8].
  • Oct 1, 2026: current weight-loss program 2026 P 1114-23 takes effect [2].
  • August 2026: weight-loss program revised to remove references to regulatory requirements; the current version says Wegovy tablets are "typically excluded" [2].
  • July 1, 2026: diabetes program 2026 P 1332-8 takes effect [3], and the Medicare GLP-1 Bridge opens to Part D members, including UHC Medicare Advantage drug plans [12].
  • May 2026: Foundayo and Wegovy HD added to the weight-loss criteria [2].
  • April 30, 2026: Optum Rx announces Weight Engage [10].
  • April 14, 2026: Wegovy HD added to Optum Rx Select and Premium at Tier 2 [6].
  • February 2026: Wegovy tablet added and criteria split by use [2].
  • November 2025: Wegovy MASH coverage added [2].
  • May 2025: Wegovy first approval set at 5 months [2].
  • March 2025: Zepbound sleep apnea coverage added [2].

How to check your UHC plan

  1. Call the pharmacy number on the back of your member card, or sign in to your UHC account, and ask whether your plan includes the weight-loss drug benefit. That one answer decides most Wegovy and Zepbound questions [1] [2].
  2. Name the drug and the reason: weight loss, heart disease, MASH, sleep apnea or type 2 diabetes. Each is judged separately [1].
  3. Ask for the prior authorization criteria in writing and the copay tier. If your employer uses an Optum Rx standard formulary, the tier is in the Premium or Select booklet [4] [5].
  4. Have your prescriber send the records the criteria ask for: BMI, related conditions, a sleep study or diabetes labs [2] [3].
  5. If you work for a large employer, ask HR whether it chose the weight-loss option for next year. Coverage can change at each plan year.

Other insurers handle this differently: see Aetna, Cigna and Blue Cross Blue Shield, or the full insurer coverage tracker.

If UnitedHealthcare says no

  • Read the reason. A benefit exclusion means your plan was not built to pay for that use; ask the plan whether any exception process exists. A criteria denial (missing BMI, no 5% loss) can often be fixed with records [2].
  • Appeal with your prescriber. Use the deadline and address on the denial letter. If you have heart disease, MASH or sleep apnea, make sure the request names that use, since standard plans cover it [1].
  • Use a manufacturer savings card if a commercial UHC plan covers it. Novo's card can bring covered Wegovy to as little as $25, with savings up to $100 a month [14]; Lilly's savings card can bring covered Zepbound to as little as $25 [16].
  • Pay the self-pay price. As of Oct 17, 2026: Wegovy pens $349 a month on NovoCare ($199 for each of a new self-pay patient's first two fills at the starting doses, through Dec 31, 2026), Wegovy HD $399, and the Wegovy pill $149 to $299 [14]. Zepbound on LillyDirect is $299 to $449 per 28 days; the $449 price for 7.5 mg and up needs a refill within 45 days, or it rises to as much as $699 [15]. More on Wegovy cost, Zepbound cost and the GLP-1 price tracker.

UHC Medicare Advantage and AARP plans

Medicare Part D, including UHC and AARP Medicare Advantage drug plans, does not cover GLP-1s for weight loss under the regular benefit. Since July 1, 2026, the Medicare GLP-1 Bridge covers Wegovy, Foundayo and the Zepbound KwikPen for weight management at $50 a month through Dec 31, 2027, for people who meet its BMI rules [12] [13]. People with type 2 diabetes, moderate to severe sleep apnea or MASH are not eligible for the Bridge; their own Part D plan may cover the drug for that condition [12]. We did not check UHC's 2026 Part D drug lists for Wegovy or Zepbound. Details: Medicare and GLP-1s.

Before you start any of these drugs

Wegovy, Ozempic, Zepbound and Mounjaro carry a boxed warning about thyroid C-cell tumors seen in rodents; do not use them with a personal or family history of medullary thyroid carcinoma or MEN 2 [19] [20]. Get care right away for severe stomach pain that does not go away or signs of an allergic reaction. Taking them with insulin or a sulfonylurea raises the chance of low blood sugar. Tell your prescriber if you are pregnant or planning to be. For an emergency, call 911.

Questions people ask

Does UnitedHealthcare cover Wegovy for weight loss?

Only if your employer bought UHC's weight-loss benefit. On standard UHC commercial plans, Wegovy for weight loss is a benefit exclusion, while Wegovy injection stays covered for heart risk and MASH [1]. Plans that bought the benefit require a BMI of 30 or more, or 27 or more with a weight-related condition, under program 2026 P 1114-23, effective Oct 1, 2026 [2].

Does UnitedHealthcare cover Zepbound?

For sleep apnea, yes on standard commercial plans: UHC's drug list reads "Benefit exclusion. Coverage for OSA only." [1] For weight loss, only on plans that bought the weight-loss benefit, with a first approval of 6 months and renewal after at least 5% weight loss [2].

Does UnitedHealthcare cover Ozempic?

Yes for type 2 diabetes, with prior authorization and records confirming the diagnosis, such as an A1C of 6.5% or more; approvals last 12 months and no metformin trial is required (program 2026 P 1332-8, effective July 1, 2026) [3]. UHC's policy says weight-loss use is "typically excluded from benefit coverage" [3].

Does UnitedHealthcare cover Mounjaro or tirzepatide?

Mounjaro is covered for type 2 diabetes under the same prior authorization as Ozempic, approved for 12 months [3]. For weight loss, the tirzepatide product is Zepbound, which standard UHC plans cover only for sleep apnea [1].

Does Optum Rx cover Wegovy?

Optum Rx lists Wegovy at Tier 2 with prior authorization on its 2026 and 2027 Premium and Select formularies, but flags it "++", meaning coverage depends on whether the plan includes weight-loss drugs [4] [7]. Check your plan's benefit, not just the formulary.

Does UnitedHealthcare cover the Wegovy pill?

UHC's weight-loss program says "Wegovy tablets are typically excluded from coverage" [2]. Self-pay on NovoCare is $149 to $299 a month by dose, as of Oct 17, 2026 [14].

Does AARP UnitedHealthcare Medicare cover Wegovy?

Not for weight loss through the plan itself. Since July 1, 2026, eligible Part D members, including Medicare Advantage drug plan members, can get Wegovy, Foundayo or the Zepbound KwikPen for $50 a month under the Medicare GLP-1 Bridge through Dec 31, 2027 [12] [13].

What does UHC need to renew a weight-loss approval?

At least 5% weight loss from your starting weight (4% for Saxenda) plus continued lifestyle changes; renewals last 12 months, with one extra 6-month approval if you have not reached the maintenance dose (program 2026 P 1114-23) [2].
