It requires a prescription for an FDA-approved use, which for Ozempic means type 2 diabetes, and it excludes Medicare, Medicaid and other government plans [1] [3].
The Ozempic savings card is Novo Nordisk's copay offer that lowers a commercially insured patient's out-of-pocket cost for Ozempic prescribed for an approved use.
Ozempic savings offers in 2026
Novo Nordisk runs two offers for Ozempic: a $25 savings offer for people whose commercial plan covers it, and a self-pay offer for everyone else with a prescription for an approved use [2] [4]. Both are tied to Ozempic's label [1] [4]. For every price channel, see Ozempic cost.
| Your situation | You pay | Limits | Source |
|---|---|---|---|
| Commercial plan covers Ozempic | As little as $25 | Savings up to $100 / $200 / $300 per 1-, 2- or 3-month fill; up to a 90-day supply | [1] |
| Commercial plan does not cover it, or no insurance | $349 a month (0.25, 0.5 or 1 mg); $499 (2 mg) | Self-pay; does not count toward a deductible | [1] [2] |
| New to the self-pay offer, 0.25 or 0.5 mg | $199 a month | First two 28-day fills, filled by Dec 31 2026 | [1] [2] |
| Ozempic tablets, self-pay | $149 (1.5 mg), $199 (4 mg) or $299 (9 mg) | 30-tablet bottle | [1] [2] |
| Medicare, Medicaid or other government plan | Not eligible for the $25 offer; may pay the self-pay price | Outside insurance | [1] [4] |

Who qualifies for the $25 card
- Commercial insurance that covers Ozempic injection 0.5 mg, 1 mg or 2 mg, or Ozempic tablets 1.5 mg, 4 mg or 9 mg [1].
- A prescription "for an FDA-approved indication" with a valid prescriber ID [1]. Ozempic's approved uses are all in adults with type 2 diabetes [3].
- Age 18 or older and a US resident [5].
- Not enrolled in Medicaid, Medicare, VA, DOD, TRICARE or a similar federal or state program. The terms say FEHB, Affordable Care Act exchange plans and state employee plans do not count as government programs [1].
- Not valid if your plan already pays the entire cost, or if your insurer uses an accumulator or maximizer program [1]. Massachusetts availability depends on state law [1].
Can you use the Ozempic coupon for weight loss?
The terms say no. Patients "may only participate in this program if they have been prescribed Ozempic for an FDA-approved indication" [1], and Ozempic has no weight loss indication [3]. Wegovy is the semaglutide approved for weight loss, with its own $25 offer and $199 to $349 self-pay prices; see the Wegovy savings offer and Ozempic vs Wegovy.
How to activate it
- Text BEGIN to 21848, or fill out Novo's four-step online form [5].
- Show the offer at your pharmacy with your prescription, or have your prescriber send it to NovoCare Pharmacy for free home delivery [5] [2].
- Your activation stays valid for up to 48 months from enrollment, unless Novo changes the program [1].
Medicare and Medicaid
The $25 offer excludes Medicare and Medicaid [1]. Government-insured patients can choose to self-pay outside their insurance [4]. Medicare's GLP-1 Bridge for weight loss does not cover Ozempic [6]. See Ozempic insurance and GLP-1s and Medicare.
Getting the prescription
Any licensed prescriber can write it. If you use a telehealth program, its fee is separate from the drug. How that works is on Ozempic online.

Ozempic safety
Ozempic has a boxed warning for thyroid C-cell tumors seen in rodents; do not use it with a personal or family history of medullary thyroid carcinoma or MEN 2. Low blood sugar is more likely with insulin or a sulfonylurea [3]. Severe stomach pain that will not go away needs prompt care. Call 911 for signs of a severe allergic reaction.
