How we check

Is Triad Rx pharmacy legit?

Triad Rx is a real, state-licensed pharmacy in Daphne, Alabama, but FDA sent it a warning letter in 2019 over compounding and sterile practices, closed that letter in 2023, and referred a 2022 inspection to the Alabama board [1] [2] [3].

MEDVi lists Triad Rx as one of its partner pharmacies [4]. We rate telehealth companies, not pharmacies, so this page states the record and no verdict.

Triad Rx is an independent community and compounding pharmacy in Daphne, Alabama.

What Triad Rx is

Triad Rx calls itself an independent community pharmacy that also compounds custom medicines, and says it has served Daphne since 2012 [5]. Its address is 26258 Pollard Road, Daphne, AL 36526 [5] [4]. Its home page lists weight loss medicines among its specialties but does not name semaglutide or tirzepatide [5].

503A or 503B?

Triad Rx is not on FDA's list of registered 503B outsourcing facilities (checked Oct 24, 2026) [6]. FDA's letters treat it as a state-licensed pharmacy compounding under section 503A, and the state board of pharmacy is its day-to-day regulator [3] [7].

Alabama license

FDA's 2023 referral letter, sent to the Alabama State Board of Pharmacy, calls Triad Rx "a pharmacy you licensed" at the Daphne address [3]. We could not pull a license record from the Alabama board's online search tool on Oct 24, 2026, so we do not list a license number or expiration date. You can search it yourself from the state links on FDA's BeSafeRx page [8].

A Form 483 lists an inspector's observations. It is not a final FDA finding. More letters are on our FDA warning letter tracker.

One point to note: the 2023 closeout says Triad had stopped making sterile drugs at that time [2]. Injectable GLP-1s are sterile products. MEDVi does not say which products Triad fills for it [4], and we do not know.

Which providers we review use Triad Rx

MEDVi names Triad Rx, RedRock Pharmacy and Beaker Pharmacy and Compounding as partner pharmacies [4]. FDA sent MEDVi a warning letter on Feb 20, 2026, in part because its product labels showed MEDVi's name, which FDA said suggests MEDVi is the compounder "when in fact it is not" [10]. Read our MEDVi review. See also Red Rock Pharmacy and Beaker Pharmacy.

What this means for you

Any drug Triad Rx compounds is not FDA-approved. FDA says it does not check the safety, effectiveness or quality of compounded drugs before they are sold [7]. That holds for compounded semaglutide and compounded tirzepatide too.

  1. Read the label. The pharmacy name and address on the vial or box should match a pharmacy you can look up. FDA lists a pharmacy name on the label that you think might be fraudulent as a red flag [11].
  2. Check the license with the state board. FDA's BeSafeRx page links every state board of pharmacy license search [8]. Look up the pharmacy in its home state and in the state where you live.
  3. Check the website with NABP. The National Association of Boards of Pharmacy runs a Safe Site Search on Safe.Pharmacy and says a safe online pharmacy accepts only valid prescriptions, is licensed where it is located and where you live, and has a pharmacist you can reach [12].
  4. Never buy research-use-only vials. They are not legal for human use and are not pharmacy products.
  5. Report problems. FDA asks people to report side effects or quality problems with any medicine to MedWatch [11]. For a medical emergency, call 911.
