How we check

Medical weight loss in Nashville

Nashville is one of the few places where Medicaid helps pay for weight loss drugs: TennCare has covered Wegovy and Zepbound as preferred drugs since August 1, 2025, with prior authorization and BMI rules [1]. A Nashville patient outside TennCare who pays the drug makers directly spends $149 to $449 a month on a brand GLP-1, based on prices we checked on Oct 17, 2026 [2] [3].

Medical weight loss is obesity treatment overseen by a licensed clinician, often using FDA-approved anti-obesity medicines along with diet and activity changes.

TennCare changes the math

Most state Medicaid programs do not pay for GLP-1 drugs prescribed for obesity. KFF counted only 13 that did in January 2026 [7], and Stateline's April 2026 report names Tennessee among them [8]. TennCare's provider notice lists Wegovy and Zepbound as preferred agents effective August 1, 2025. Prior authorization is required, and the member needs a BMI of 30 or higher, or 27 or higher with a weight-related condition [1].

If you have TennCare, start with your primary care provider and ask them to submit the prior authorization. Coverage rules can change: Stateline reported that several states were weighing cuts in 2026 [8].

Davidson County figures

CDC PLACES puts adult obesity at 32.0% in Nashville and 32.3% across Davidson County for 2023, with diagnosed diabetes at 10.9% and 11.2% [4] [5]. Nashville's consolidated metro government had 721,074 residents in mid-2025 [6].

If you pay cash

Manufacturer cash prices a Nashville prescription can use (checked Oct 17, 2026)

Zepbound vial or KwikPen, LillyDirect$299 to $449 [3]Dose sets the price; a late refill costs more

Wegovy tablets, NovoCare$149 to $299 [2]1.5 mg is $149, 4 mg is $199, 9 or 25 mg is $299

These are drug prices alone; a Nashville clinic bills its visits and labs separately. Every channel is on the GLP-1 price guide.

Lilly and Novo Nordisk set these cash prices nationally, so the same order costs the same in Nashville as in Memphis. The local difference is the program fee a clinic adds. Drug-specific guides: tirzepatide in Nashville and semaglutide in Nashville.

When TennCare does not apply

  • Medicare Part D. The GLP-1 Bridge runs until December 31, 2027 and sets a $50 monthly copay on Wegovy, Foundayo or the Zepbound KwikPen for enrollees who qualify [11] [12].
  • Job-based plans. Lilly, citing an employer-benefits survey, says roughly half of people with employer insurance have no obesity drug coverage [13]. Where a plan does cover Zepbound, Lilly's card can bring the copay down to $25 [14].
  • Sleep apnea. Zepbound also carries an approval for moderate to severe obstructive sleep apnea in adults with obesity, and some insurers handle that use separately [15].

Picking a Nashville prescriber

Look the clinician up with the Tennessee Board of Medical Examiners first [16]. Then ask whether the prescription will be a brand product or a compounded copy. Compounded semaglutide and tirzepatide have no FDA approval, and FDA does not check them for safety, effectiveness or quality [17]. For telehealth, our online program comparison and the FDA warning letter list help sort sellers.

Safety

Label warnings for Wegovy and Zepbound

People with a personal or family history of medullary thyroid carcinoma or MEN 2 should not take either drug. Stomach pain that is severe and keeps going may be pancreatitis: stop the drug and get seen. The labels also list gallbladder problems, kidney injury from dehydration, and more low blood sugar when insulin or a sulfonylurea is in the mix. Both say to stop once a pregnancy is recognized [18] [19]. Facial swelling or trouble breathing is a 911 call.

Questions people ask

Will TennCare pay for Zepbound or Wegovy in Nashville?

It can. TennCare's provider notice made Wegovy and Zepbound preferred obesity drugs on August 1, 2025. Prior authorization is required, and members qualify with a BMI of 30 or more, or 27 or more plus a weight-related condition [1].

What does a Nashville patient pay for a GLP-1 with no coverage?

Maker cash prices checked Oct 17, 2026 start at $149 a month for the lowest Wegovy tablet, $349 for the Wegovy pen, and $299 to $449 for Zepbound [2] [3]. A clinic's own fee is extra.

How does Nashville's obesity rate compare with Davidson County's?

They are close. CDC PLACES puts 2023 adult obesity at 32.0% inside the city and 32.3% for Davidson County as a whole [4] [5].

Is Tennessee unusual in covering these drugs through Medicaid?

Yes. KFF found just 13 state Medicaid programs paying for obesity GLP-1s under fee-for-service in January 2026, down from 16 in October 2025 [7], and Stateline lists Tennessee among them [8].

Where do I confirm a Nashville prescriber's Tennessee license?

Search the Tennessee Board of Medical Examiners before the first appointment [16].

What did semaglutide do in its main obesity trial?

Adults on semaglutide 2.4 mg in the STEP 1 trial lost 14.9% of body weight on average by week 68, against 2.4% for placebo [20].
