How we check

Does Aetna cover Wegovy, Zepbound, Ozempic or Mounjaro?

Aetna covers Wegovy and Zepbound for weight loss only when your plan includes weight-loss drugs, and Aetna itself says "Many Aetna benefit plans specifically exclude coverage of weight reduction medications" [1]. Where the benefit exists, CVS Caremark's criteria ask for 6 months in a weight management program and a BMI of 30, or 27 with a weight-related condition [2] [3]. Ozempic and Mounjaro are covered for type 2 diabetes with prior authorization and lab proof of the diagnosis under the diabetes GLP-1 policy Aetna publishes (5694-D) [6] [7].

Aetna is the health insurer owned by CVS Health; its pharmacy benefits run through CVS Caremark, so Caremark's formularies and prior authorization criteria decide most Aetna GLP-1 requests [1] [10].

Caremark's default Zepbound criteria exclude Zepbound vials and KwikPens, pointing to Lilly's LillyDirect program for those [3]. Whether Wegovy for heart risk or Zepbound for sleep apnea is covered on an Aetna plan that excludes weight loss is not stated in the documents we read [4]. Both the Caremark default Wegovy criteria and the state-specific policy also include criteria for Wegovy injection for MASH [2] [4]. More: CVS Caremark coverage.

Why many Aetna members are excluded

Weight-loss drug coverage at Aetna is a choice the employer or plan makes, not a default. Aetna's own policy bulletin says many of its plans exclude these drugs [1]. Employer surveys show how uneven that choice is: in KFF's 2025 survey of firms that offer health benefits, 43% of firms with 5,000 or more workers covered GLP-1s primarily for weight loss, compared with 16% of firms with 200 to 999 workers [20].

If you bought an individual plan, the answer changed in 2026: Aetna left the ACA exchanges for 2026, affecting about 1 million members in 17 states [8]. People who had Aetna marketplace coverage in 2025 are now with another insurer, whose rules apply. Compare Blue Cross Blue Shield, UnitedHealthcare and Cigna, or the full insurer coverage tracker. The criteria Caremark applies for Aetna

Two sets of criteria show up for Aetna members. Caremark's default forms for Wegovy (4774-C) and Zepbound (6947-C) use the same core rules as the July 1, 2025 Zepbound policy Aetna published for non-Medicare plans [2] [3] [5]. Aetna plans in certain states use a stricter "Specific States" policy (5098-C; the file name reads 5089-C), which does not name the states [4].

For sleep apnea, Caremark's Zepbound criteria require an apnea-hypopnea index (AHI) of 15 or more and a current BMI of 30 or more, with a 6-month first approval and 12-month renewals [3]. Caremark's default Wegovy criteria and the state-specific policy both list Wegovy for heart risk for adults with established heart disease who do not have type 2 diabetes and are on standard heart therapy [2] [4]. The state policy also requires a BMI of 27 or more and points people with diabetes to Ozempic for heart benefit [4].

Ozempic and Mounjaro on Aetna

Aetna's diabetes GLP-1 policy, 5694-D, covers Ozempic, Rybelsus, Trulicity, Victoza, Byetta, Bydureon BCise and Mounjaro for type 2 diabetes [6]. Approval needs a type 2 diabetes diagnosis plus documentation of one of: an A1C of 6.5% or higher, a 2-hour glucose of 200 or higher on a glucose tolerance test, symptoms of high blood sugar with a random glucose of 200 or higher, or a fasting glucose of 126 or higher [6] [7]. Approvals last 36 months. There is no metformin trial requirement, and Caremark's version lets the claim pay without prior authorization when pharmacy records already show metformin or certain other diabetes drugs [7].

The version on Aetna's site is dated May 2023 [6]; Caremark's current default version (P05-2025) has the same criteria [7]. We could not confirm that every Aetna plan uses the 2025 version. The policy offers no route for weight loss or prediabetes alone; for weight loss, the plan looks at Wegovy or Zepbound instead. See Ozempic insurance and Ozempic vs Mounjaro.

Timeline: Aetna and Caremark changes

  • Oct 1, 2026: Zepbound returns to Caremark commercial template formularies as "an additional preferred option" [10]. Members switching need a new prescription and prior authorization, as one plan sponsor, NRECA, told its members [12].
  • July 1, 2026: the Medicare GLP-1 Bridge starts for Part D members, including Aetna Medicare Advantage drug plans [13].
  • June 1, 2026: Caremark removes its new-to-market block on Foundayo [10].
  • May 28, 2026: CVS announces the Zepbound return [10]; Wegovy injection and pill keep preferred status [11].
  • March 10, 2026: Aetna last reviews its weight-loss policy bulletin, CPB 0039 [1].
  • Jan 1, 2026: Aetna no longer sells ACA exchange plans [8].
  • July 1, 2025: Caremark templates prefer Wegovy and remove Zepbound [9].
  • May 1, 2025: CVS announces the exchange exit [8].

Checking your Aetna benefits

  1. Call the pharmacy number on your Aetna card and ask one question first: does my plan include weight-loss drugs? If not, a weight-loss request is likely to be denied whatever your BMI [1].
  2. Ask which criteria apply: Caremark default or the state-specific policy with a BMI 40 floor [2] [4].
  3. Ask about the 6-month program. Keep records of any dietitian, program or clinic visits, since the default criteria require them before the drug [2].
  4. For Zepbound, ask whether your plan adopted Caremark's Oct 1, 2026 change; sponsors can customize [10].
  5. For Ozempic or Mounjaro, have your prescriber attach the lab result that confirms type 2 diabetes [6].

When Aetna denies the request

Start with the denial letter: it says whether the plan excludes the drug or you did not meet a criterion. A missing program record or BMI can be fixed and resent. Appeal with your prescriber by the date on the letter. On plans that kept Wegovy as the only preferred drug, CVS said in 2025 that members who had intolerable side effects on Wegovy, or did not lose enough weight, could request a formulary exception for a different weight-loss drug, reviewed case by case [9].

Self-pay prices as of Oct 17, 2026: Wegovy pens $349 a month, Wegovy HD $399 and the Wegovy pill $149 to $299 on NovoCare, with $199 for each of a new self-pay patient's first two starting-dose pen fills through Dec 31, 2026 [15]. Zepbound on LillyDirect costs $299 to $449 per 28 days; the $449 price for 7.5 mg and up needs a refill within 45 days, or it rises to as much as $699 [16]. If an Aetna commercial plan covers the drug, manufacturer cards can cut the copay to as little as $25 [15] [17]. Ozempic self-pay is $349 a month, or $499 for the 2 mg dose [18]. Compare channels on Wegovy cost, Zepbound cost and the GLP-1 price tracker.

Aetna Medicare members

Aetna Medicare Advantage drug plans follow federal Part D rules, which exclude drugs used only for weight loss. Since July 1, 2026, the CMS Medicare GLP-1 Bridge covers Wegovy, Foundayo and the Zepbound KwikPen for weight management at $50 a month through Dec 31, 2027, for people 18 or older who meet its BMI rules [13] [14]. People with type 2 diabetes, moderate to severe sleep apnea or MASH are not eligible; their Part D plan may cover a GLP-1 for that condition [13]. We did not check Aetna's 2026 Part D drug lists. More on Medicare and GLP-1s.

Safety, whoever pays

Semaglutide (Wegovy, Ozempic) and tirzepatide (Zepbound, Mounjaro) carry a boxed warning for thyroid C-cell tumors in rodents and should not be used with a personal or family history of medullary thyroid carcinoma or MEN 2 [21] [22]. Severe stomach pain that will not go away can signal pancreatitis; get care right away, and also for signs of an allergic reaction. With insulin or a sulfonylurea, watch for low blood sugar. Tell your prescriber about pregnancy plans. In an emergency, call 911.

Questions people ask

Does Aetna cover Wegovy for weight loss?

Only if your plan includes weight-loss drugs; Aetna says many of its plans exclude them [1]. Where covered, Caremark's criteria require at least 6 months in a weight management program and a BMI of 30 or more, or 27 or more with a weight-related condition, with renewal after 5% loss [2].

Does Aetna cover Ozempic?

For type 2 diabetes, yes with prior authorization: Aetna's policy 5694-D needs a diabetes diagnosis documented by an A1C of 6.5% or more, a fasting glucose of 126 or more, or a similar test, and approves for 36 months [6] [7].

Does Aetna cover Ozempic for weight loss?

Aetna's diabetes policy for Ozempic covers type 2 diabetes and lists no weight-loss route [6] [7]. For weight loss, the plan looks at Wegovy, which is covered only on plans that include weight-loss drugs [1].

Does Aetna cover Mounjaro?

Mounjaro is in Aetna's diabetes GLP-1 policy, 5694-D, and is approved for 36 months with documented type 2 diabetes [6] [7]. Without diabetes, the tirzepatide product for weight loss or sleep apnea is Zepbound.

Does Aetna cover Zepbound or tirzepatide in 2026?

CVS Caremark removed Zepbound from its template formularies on July 1, 2025 and added it back as an additional preferred option on Oct 1, 2026 [9] [10]. Your plan must include weight-loss drugs, and sponsors can customize [10]. For sleep apnea, Caremark's criteria need an AHI of 15 or more and a BMI of 30 or more [3].

Does Aetna cover the Wegovy pill?

Caremark's Wegovy criteria include the tablet, with a 6-month first approval, on plans that cover weight loss [2]. Wegovy injection and pill keep preferred status on Caremark formularies [11]. Self-pay is $149 to $299 a month on NovoCare, as of Oct 17, 2026 [15].

Does Aetna Medicare cover Wegovy?

Not through the regular Part D benefit for weight loss. Since July 1, 2026, eligible Part D members, including Aetna Medicare Advantage drug plan members, can get Wegovy, Foundayo or the Zepbound KwikPen for $50 a month through the Medicare GLP-1 Bridge until Dec 31, 2027 [13] [14].

Does Aetna cover Wegovy for sleep apnea?

Sleep apnea is not an approved Wegovy use [21]. The GLP-1 approved for sleep apnea is Zepbound, and Caremark's criteria for it require an AHI of 15 or more and a BMI of 30 or more [3].

How do I get Wegovy covered by Aetna?

Confirm your plan includes weight-loss drugs [1], then document at least 6 months in a weight program and a BMI of 30, or 27 with a related condition, and have your prescriber submit the Caremark request [2]. If your plan uses Aetna's state-specific policy, the BMI floor is 40, or 35 with a condition [4].
