A medical weight loss program is prescriber-led care for obesity that can include FDA-approved drugs, nutrition support and follow-up labs.
Bexar County's weight and diabetes picture
San Antonio had 1,548,422 residents in the Census Bureau's July 2025 estimate [4]. In Bexar County, CDC PLACES estimates that a bit over a third of adults (34.1%) had obesity in 2023, and 13.9% had been told they have diabetes [3]. That diabetes figure sits above Dallas County's 12.5% [6], which matters because some drugs and some insurance rules depend on a diabetes diagnosis.

The lowest-cost brand starting points
With half of San Antonio households earning under $65,056 [4], the entry price matters. Here is what the first months cost when bought directly from the makers.
| Medicine (form) | First dose and price | Where it tops out | Ref |
|---|---|---|---|
| Foundayo (daily pill) | 0.8 mg at $149 | $299 for 9 mg and above; 2.5 mg $149 and 5.5 mg $199 are temporary prices through 12/31/2026 | [2] |
| Wegovy (daily pill) | 1.5 mg at $149 | $299 for 9 or 25 mg | [1] |
| Wegovy (weekly pen) | 0.25 mg at $199 for a new buyer's first two fills | $349 for every standard dose | [1] |
| Zepbound (weekly vial or pen) | 2.5 mg at $299 | $449 at 7.5 to 15 mg with on-time refills | [7] |
Cheaper is not the same as equal. In Foundayo's main trial, adults without diabetes on the 17.2 mg dose lost an average of 11.1% of body weight at 72 weeks, against 2.1% on placebo [8]; tirzepatide's SURMOUNT-1 trial reported larger averages [9]. Compare the drugs in Foundayo versus the Wegovy pill and the full GLP-1 price list.
Coverage options for San Antonio households
- Texas Medicaid: no GLP-1 coverage for obesity, per KFF's January 2026 review [5].
- Medicare: the GLP-1 Bridge covers Foundayo, Wegovy (pen or pill) and the Zepbound KwikPen at $50 a month from July 2026 through 2027 for those who qualify. Zepbound vials and single-dose pens are not on the list [10].
- Job-based plans: about half of workers with employer insurance lack obesity-drug coverage, Lilly says, citing IFEBP [11]. If your plan does cover a brand, maker savings cards can bring the copay to $25 [1][12].

Finding care in San Antonio
University Health is one of the San Antonio health systems in our research [13]. We did not find a published nonsurgical weight program or price on its site, so call and ask what it offers and what self-pay costs. For any program, confirm the prescriber on the Texas Medical Board website [14], ask whether the drug is a brand product or compounded, and get the total price in writing. FDA says compounded semaglutide and tirzepatide are not FDA-approved and are not reviewed by FDA for safety, effectiveness or quality [15].
Warning signs on these drugs
Seek care if stomach pain becomes severe and does not let up, since the labels list pancreatitis as a risk, or if a rash, swelling or wheezing suggests an allergic reaction. Anyone also taking insulin or a sulfonylurea has a higher chance of low blood sugar. Every GLP-1 drug in the table carries a boxed warning about thyroid C-cell tumors [16][17][8]. In an emergency, call 911.
