Who qualifies

  • Commercial drug insurance, either covering Foundayo (the $25 tier) or not covering it (the $149 to $299 tier) [1].
  • Not enrolled in any state, federal or government-funded plan, including Medicaid, Medicare, Medicare Part D, Medicare Advantage, Medigap, DoD, VA and TRICARE/CHAMPUS [1].
  • Card activation is required [1].
  • Not valid for Massachusetts residents if an AB-rated generic is available, or California residents if an FDA-approved therapeutic equivalent is available [1].
  • Lilly can change or end the offer at any time [1].

The self-pay card for people without insurance

Lilly's Foundayo self-pay savings card "is for self-paying (cash) patients only" [1]. It uses the same prices as the not-covered tier, ends Dec 31 2026, and those payments cannot be sent to insurance or counted toward a deductible [1]. You can also order through LillyDirect: your prescriber sends the prescription to LillyDirect Pharmacy, and you choose free delivery or retail pickup [2]. See Foundayo cost for the full picture.

How it compares with Zepbound's and Wegovy's cards

Foundayo's covered-plan cap is lower than Zepbound's: $1,000 a year and 10 fills, against $1,300 and 13 fills for the Zepbound pen [1] [6]. Wegovy's offer caps savings at $100 per 1-month fill with no annual cap printed on its terms page [7]. Details: Zepbound savings card and Wegovy savings offer. The two GLP-1 pills are compared on Foundayo vs Wegovy pill.

Medicare and Foundayo

The savings card excludes Medicare [1]. The Medicare GLP-1 Bridge covers Foundayo for eligible Part D members with a $50 copay for a one-month supply, from July 1 2026 through Dec 31 2027 [4] [5]. Lilly links to its Medicare page from LillyDirect [2]. See GLP-1s and Medicare.

Foundayo safety

Foundayo has a boxed warning: other GLP-1 drugs caused thyroid C-cell tumors in rodents, and the human relevance is not known. Do not use it with a personal or family history of medullary thyroid carcinoma or MEN 2. If you take birth control pills, switch to a non-oral method or add a barrier method for 30 days after starting and after each dose increase [9]. Severe stomach pain that will not go away needs prompt care. Call 911 for signs of a severe allergic reaction.
