An FDA advisory committee voted 8 to 5, with 1 abstention, on July 24, 2026 to recommend allowing it, but FDA has not acted [2] [3]. Semax is a registered drug in Russia, sold there as nasal drops, and FDA's review flagged a possible bleeding risk and possible abuse potential [4].
Semax is a synthetic seven-amino-acid peptide based on a fragment of the hormone ACTH, registered as a nasal drug in Russia and not approved in the US [4].
Legal status of Semax
Waiting on FDA after a favorable voteSemax won a favorable but nonbinding advisory committee vote in July 2026, and FDA has not acted on it [3]. It still cannot be legally compounded or sold as a drug [1].
- FDA status
- FDA removed it from Category 2 in April 2026 because the nominators withdrew their nominations [6]. That removal did not make it legal to compound [7]. FDA now lists it as "nominated but withdrawn," which is outside every category [15]. It is not in Category 1 and is not on the 503A bulks list, so a 503A pharmacy may not lawfully compound it today [1]. FDA's briefing flags a bleeding risk and possible abuse potential, and it found too little evidence for all three proposed uses [4].
- Advisory committee
- On July 24, 2026, the committee voted 8 yes, 5 no and 1 abstain to recommend adding Semax to the 503A bulks list [2]. The committee reviewed it for cerebral ischemia, migraine and trigeminal neuralgia [8]. FDA staff had recommended against listing it [4]. FDA has not posted its own vote count on the meeting page [8]. Advisory committee votes are not binding. Nothing changes until FDA acts [9].
- FDA-approved products
- None in the US. We found no FDA-approved drug that contains Semax. It is a registered drug in Russia, sold there as nasal drops [4].
- Sports (WADA)
- Semax is not named on the 2026 list [12]. The S0 class covers substances with no approval by any government health authority [12]. Semax is a registered drug in Russia [4], so whether S0 covers it is unclear. Athletes should ask their anti-doping agency before use.
- What happens next
- To add it to the 503A bulks list, FDA must go through notice-and-comment rulemaking, starting with a proposed rule. FDA could also place it in Category 1 or announce enforcement discretion in the meantime [9]. As of Oct 16, 2026, FDA had not started rulemaking or moved any of these peptides to Category 1 [3]. HHS has sent an "Interim Access Policy for Compounding Using Certain Peptides" to the White House budget office for review. Holland & Knight dates that submission Oct 14, 2026 [3], while the AP reported it appeared on a government website on Oct 15 [10]. The policy has not been issued and its text is not public [3]; reporting on Oct 21, 2026 still described it as under review [17]. Reports say it would cover some popular peptides, but we could not confirm whether Semax is one of them [10]. One law firm says there is no reliable timetable for final action [9]. A drug safety group petitioned FDA on Oct 16, 2026 not to allow it [11].

Semax legal status in the US
A drug approved in another country is not approved here. Semax has no FDA approval, and a 503A pharmacy may not compound it because it has no USP monograph, is not in an approved US drug and is not on the bulks list [5] [1]. FDA removed it from Category 2 in April 2026 after the nominators withdrew, a step that did not make it legal [6] [7].
FDA reviewed it anyway, on its own initiative, for three uses: cerebral ischemia (reduced blood flow to the brain), migraine and trigeminal neuralgia, a facial nerve pain [8]. The committee voted 8-5-1 to recommend it on July 24, 2026 [2]. FDA staff had concluded that the evaluation criteria "weigh against" listing it [4].
What could change the answer: an FDA proposed rule, Category 1 placement or a statement of enforcement discretion [9]. An HHS interim peptide access policy was under White House review in October 2026, with no public list of covered peptides [3] [10]. A citizen petition filed Oct 16, 2026 asks FDA not to allow Semax or six other peptides [11].

A Russian nasal drug, not a US one
FDA describes Semax as "a synthetic heptapeptide analog of the ACTH 4-10 fragment," with the sequence Met-Glu-His-Phe-Pro-Gly-Pro [4]. ACTH is a pituitary hormone. In Russia it "is a registered drug ... available as 0.1% and 1% nasal drops" [4]. The US nominations proposed a nasal spray or an injection under the skin [4].
Russian registration means a foreign regulator accepted it under its own rules. It does not mean FDA has reviewed the studies, and FDA's own review reached a negative conclusion on effectiveness.
What the human studies covered
Unlike several peptides on the same agenda, Semax has been given to people. FDA's briefing counts:
- 33 to 47 healthy adults [4].
- 69 adults with medical conditions [4].
- 451 children with depression or tics and Tourette syndrome [4].
Most of this work is Russian-language research. Even so, FDA wrote that "there is insufficient evidence to support the effectiveness" of Semax for any of the three uses it reviewed [4]. Searches for "Semax benefits" often turn up claims about focus and memory; none of those was a use FDA reviewed, and we found no FDA finding that supports them.
FDA's two safety flags: bleeding and abuse
Bleeding
FDA noted a paper describing possible anti-clotting properties of Semax and wrote that "this raises concern about the risk of bleeding" [4]. That matters for anyone on blood thinners or facing surgery.
Abuse potential
FDA found no studies testing whether Semax is habit-forming. It also noted that in mice, Semax "potentiated amphetamine-induced dopamine release," and that increased dopamine activity in that brain region "is a response typically induced by drugs of abuse" [4]. That is a signal to study, not proof of addiction.
If you have used Semax
Tell your doctor and dentist, especially before surgery or if you take a blood thinner, because FDA flagged a possible bleeding risk [4]. Unusual bruising, nosebleeds that will not stop or blood in urine or stool need prompt care. Call 911 for trouble breathing or signs of stroke.
Semax and drug testing
Semax is not named on the 2026 WADA Prohibited List [12]. The S0 class covers substances with no government approval for human use anywhere [12]. Because Semax is registered in Russia [4], whether S0 applies is unclear. Ask your anti-doping agency before using it.
Legal options in the US
There is no legal way to buy Semax today
"Research use only" products are not legal for personal use; FDA has said so in warning letters [13]. For migraine or nerve pain, FDA-approved treatments exist, and a doctor can go over them. See our peptide status tracker for every peptide FDA is reviewing, Selank for Semax's sister peptide, dihexa, another peptide marketed for memory, epitalon from the same July 24 session, the legal peptide price tracker, BPC-157 and wellness injections.

How we checked
We read FDA's Semax briefing and categories document and rechecked FDA's compounding and advisory committee pages on Oct 24, 2026 [1] [14]. FDA has not posted July 2026 minutes, so the vote comes from a law firm report [2]. We do not link to sellers. See our editorial standards.
