How we check

altRx review: is altRx legit?

altRx is a real company, and we rate it Use caution because FDA sent it a warning letter on June 8 2026 over its compounded GLP-1 marketing [4].

Bottom line: Rated Use caution; from $89/mo.

On Oct 24 2026 the phrases FDA cited were not on its home or product pages, and its vial images read only "Compounded GLP-1" [1] [2]. BBB does not rate it, saying the business is responding to previously closed complaints, and lists 329 complaints since May 2026, many about missed shipments, unexpected charges and cancellations that did not work [5] [6]. On Oct 24 2026 it advertised compounded semaglutide from $89 a month and compounded tirzepatide from $149 a month [1].

altRx is an online membership that connects people with prescribers at OpenLoop Healthcare Partners for compounded semaglutide and tirzepatide, plus brand Zepbound, Wegovy and Ozempic.

Our verdict

Use caution

A June 2026 FDA warning letter whose cited claims are now gone from its pages puts altRx at Use caution under our rating rules [4]. BBB does not rate it while it responds to closed complaints, and its 329-complaint file is heavy on billing, cancellation and refund problems [5] [6].

What checks out

  • Names its prescribing group, OpenLoop Healthcare Partners, and four 503A pharmacies [1].
  • Its product pages say plainly that the compounded drugs are not FDA-approved [2] [3].
  • Low advertised prices: $89 a month for compounded semaglutide and $149 for compounded tirzepatide [2] [3].

What to watch

  • 329 BBB complaints in about five months, with 87 unanswered or unresolved [6].
  • FDA warning letter dated June 8 2026 [4].
  • Payments are final and non-refundable, and multi-month plans cannot be refunded for unused months [8].
  • The home page promises "We will fix your broken metabolism" and quotes weight loss from member self-reports, not trials [1].
FDA record
Warning letter dated June 8 2026 (MARCS-CMS 728236) over "clinically proven" claims for compounded drugs and the altRx name on product labels [4]. Those exact phrases were not on its home or product pages on Oct 24 2026 [1] [2].
BBB
Not rated, not accredited; BBB says the business is in the process of responding to previously closed complaints. 329 complaints since the file opened May 5 2026; 46 unanswered and 41 unresolved, as of Oct 24 2026 [5] [6].
Best for
People who want low compounded semaglutide or tirzepatide prices and accept that all payments are final.

Why altRx is rated Use caution

Our rating rules put a company at Use caution when it had an FDA warning letter whose cited claims were fixed, with no Avoid trigger such as a BBB Pattern of Complaints alert. altRx got its letter on June 8 2026 [4]. Its BBB profile shows no alert, but BBB does not rate it while it responds to previously closed complaints [5]. altRx's BBB file opened on May 5 2026 and held 329 complaints by Oct 24 2026 [5] [6]. That is a lot for five months.

  • Missed or late medicine. Delivery issues are 51 of the 329 complaints. One person wrote on Oct 3 2026, "I was already past my scheduled injection date" [6].
  • Billing and refunds. 39 complaints are filed as billing issues, and others ask for refunds under product and order headings. One person said they were charged for a 12-month plan after agreeing to 3 months (Sept 29 2026) [6].
  • Cancelling. People say they could not cancel or change billing in the patient portal (Oct 1 2026) [6].
  • No answer. 46 complaints are unanswered and 41 unresolved. Many first replies are the same holding message [6].

BBB shows altRx as "Not Rated" because "the business is in the process of responding to previously closed complaints" [5]. Its Trustpilot page now says the business "goes against our guidelines and is no longer visible on Trustpilot" [7].

What altRx costs

altRx shows one monthly price per drug and says the price is the same at every dose, with no membership fee and shipping included [1] [2] [3]. It does not list prices for brand Zepbound, Wegovy or Ozempic on the pages we read.

A "3 month subscription" appears in a sample order screen, but no 3-month price is posted [2].

Brand drugs cost less than many people expect when bought direct: Zepbound is $299 to $449 a month on LillyDirect if you refill within 45 days [11]. See Zepbound cost and our semaglutide cost guide for every channel.

Refund and cancellation terms

  • "All payments for the subscription services are final and non-refundable at the time of purchase" [8].
  • Multi-month and bundled plans are sold as one purchase and are not refunded for unused months [8].
  • Cancel at least 72 hours before the next bill by email, phone or portal; cancelling stops future charges only [8].
  • If a provider finds you medically ineligible, altRx refunds the unused part of the current term, less services used [8].

What you get from altRx

altRx says it is a technology and admin platform and does not employ clinicians. Prescribing is done by OpenLoop Healthcare Partners, PC and its affiliated practices [1] [8]. It names four 503A compounding pharmacies: RedRock Pharmacy (St. George, UT), Health Warehouse (Florence, KY), Precision Medicine (Bellmore, NY) and Triad Rx (Daphne, AL) [1]. It says it serves all 50 states, with provider access varying by state [1].

Its own footer says the compounded drugs "are not FDA approved and have not been evaluated by the FDA for safety, efficacy, or quality" [1]. The home page still says "We will fix your broken metabolism" and quotes an 18% average loss "based on self-reported data from AltRx members" [1]. Read compounded semaglutide and compounded tirzepatide for what that status means.

FDA and complaint record

FDA: The June 8 2026 letter went to Trinity HealthCare Supply, LLC dba altRx in Miami. FDA cited the phrases "GLP-1s are clinically proven medications" and "clinically proven weight loss" for compounded drugs, and the altRx name on product labels, which FDA said implies altRx makes the drugs [4]. FDA tells telehealth sellers not to call compounded drugs clinically proven or brand them in ways that suggest they are the compounder [9]. On Oct 24 2026 neither phrase was on the home page or the two compounded product pages, and the vial images were labeled only "Compounded GLP-1" [1] [2] [3]. Both product pages now say "Weight-loss outcomes supported by peer-reviewed clinical data", while the same pages say their weight loss figures "are not derived from clinical trials or peer-reviewed studies" [2] [3]. A kit photo still shows an altRx-branded box [1]. See every letter on our FDA warning letter tracker.

BBB: Not rated and not accredited, 329 complaints, as described above [5] [6].

Who should look elsewhere

Anyone who needs medicine to arrive on time, or who may need to cancel, should weigh the complaint file before paying. People who want a brand drug can buy it direct from Lilly or Novo Nordisk and pay a telehealth program only for the visit; see tirzepatide online and semaglutide online.

Compounded GLP-1 safety

Compounded semaglutide and tirzepatide are not FDA-approved. FDA has received 990 adverse event reports for compounded semaglutide and more than 730 for compounded tirzepatide as of May 31 2026, and says not to use injectables that arrive warm [10]. The tirzepatide label carries a boxed warning for thyroid C-cell tumors seen in rodents [16]. Severe stomach pain that will not go away needs prompt care. Call 911 for signs of a severe allergic reaction.
