Medical weight loss is obesity care run by a licensed prescriber, usually pairing an FDA-approved drug with eating, activity and follow-up plans.
An independent city with a high obesity rate
Baltimore is an independent city, so it counts as its own county in federal data. CDC PLACES gives Baltimore City an adult obesity rate of 38.7% and a diagnosed diabetes rate of 14.4% for 2023 [1]. The place-level model lands almost on top of that: 38.1% and 14.4% [6]. Census counted 569,997 residents in July 2025 [5].

The coverage gap in Maryland
Maryland sits outside the group of 13 Medicaid programs that KFF found paying for obesity GLP-1s in January 2026 [2]. Half of Baltimore households earn less than $62,177 [5]. Paying the $349 Wegovy pen price for a year, $4,188 [4], would use about 6.7% of it. In a city with this income level and this obesity rate, the gap between need and coverage is wide, and comparing prices matters.
- Seniors: 15.3% of Baltimoreans are 65 or older [5]. Medicare's temporary GLP-1 Bridge lets a qualifying Part D member fill Wegovy, Foundayo or a Zepbound KwikPen at $50 per month, with the program ending December 31, 2027 [7] [8].
- Diabetes: with 14.4% of adults diagnosed [1], many Baltimoreans may be offered Ozempic or Mounjaro, which are labeled for blood sugar control and covered under different rules [9] [10].
- Work plans: a Baltimore worker whose plan lists Zepbound may pay as little as $25 with Lilly's savings card [11].
Paying cash in Baltimore
On LillyDirect, a Baltimore patient pays $299 for the 2.5 mg starting strength of Zepbound, $399 at 5 mg, and $449 from 7.5 to 15 mg when each refill comes within 45 days; miss that window and it is $499 at 7.5 mg or $699 at 10 to 15 mg [3]. Doses rise in 2.5 mg steps at least 4 weeks apart under the label [12], so plan on reaching the higher tiers. Wegovy pens cost $349 at every strength from 0.25 to 2.4 mg, with $199 for a new buyer's first two 0.25 and 0.5 mg fills through Dec 31, 2026. Wegovy tablets are $149 at 1.5 mg, $199 at 4 mg and $299 at 9 or 25 mg [4].

Screening a Baltimore program
- Confirm the prescriber's license with the Maryland Board of Physicians [13].
- Find out which pharmacy fills the drug and whether it is brand or compounded. Compounded GLP-1s lack FDA approval, and FDA does not review their safety, effectiveness or quality [14].
- Look up any online seller in our FDA warning letter tracker.
- Ask for written terms: monthly fee, what it buys, how to quit.

Medication safety
If your stomach hurts badly for hours, that can be pancreatitis, so hold the drug and be examined. A personal or family history of medullary thyroid carcinoma or MEN 2 rules these drugs out. Lows are a risk for anyone who also injects insulin or swallows a sulfonylurea. Stop Zepbound if you become pregnant [15]. Facial swelling or labored breathing needs 911.

SURMOUNT-1 enrolled adults without diabetes. After 72 weeks the average loss ran from 15.0% to 20.9% across doses, against 3.1% for placebo [16].
