How we check

Zepbound with other medicines and health conditions

Not as an approved use. Zepbound is FDA-approved for weight management and for obstructive sleep apnea in adults with obesity, not for blood sugar control; the same drug, tirzepatide, is sold for type 2 diabetes as Mounjaro [1] [2]. Zepbound was still tested in adults with type 2 diabetes: in Study 2 on its label, HbA1c fell 2.1 points vs 0.5 on placebo [3].

Medical review: pending. A licensed clinician has not yet reviewed this page.

Zepbound interactions are the medicines and health conditions, such as diabetes treatment, thyroid history and kidney problems, that the Zepbound label says change how the drug should be used.

Approved uses
Weight management in adults; sleep apnea in adults with obesity [1]
Diabetes brand
Mounjaro, for type 2 diabetes from age 10 [2]
HbA1c in type 2 diabetes
-2.1 points vs -0.5 on placebo at 72 weeks [3]
Low blood sugar with a sulfonylurea
10.3% in the diabetes trial [3]
Thyroid
Boxed warning; not for people with MTC or MEN 2 history [3]

Zepbound or Mounjaro: one drug, two labels

Zepbound contains tirzepatide, and Lilly says it should not be used with other tirzepatide products [4]. Mounjaro is the other tirzepatide product. Its label covers blood sugar control in adults and children 10 and older with type 2 diabetes, and lowering the risk of heart attack, stroke and cardiovascular death in high-risk adults with type 2 diabetes [2]. Zepbound's label covers weight reduction and sleep apnea [1]. So if diabetes treatment is the goal, Mounjaro is the labeled brand; see Mounjaro vs Zepbound and Mounjaro.

The label split shows up in coverage too. Medicare's GLP-1 Bridge for weight drugs excludes people with type 2 diabetes, who may get GLP-1 coverage through their regular Part D plan instead [5]. Our sources contain no label language on type 1 diabetes for Zepbound; anyone with type 1 diabetes should ask their endocrinologist.

Does Zepbound lower A1C?

Yes, in its trials. In Study 2, adults with type 2 diabetes started with an average HbA1c of 8.0% to 8.1%. At 72 weeks it fell 2.1 points on 10 mg and 15 mg vs 0.5 on placebo. In Study 1, adults without diabetes started at 5.6% and fell 0.4 points vs 0.1 [3]. New-onset diabetes is not among the label's warnings [3]. More trial data are on Zepbound results.

Insulin, sulfonylureas and low blood sugar

This is the interaction with the clearest numbers. In the diabetes trial, low blood sugar under 54 mg/dL occurred in 4.2% on Zepbound vs 1.3% on placebo, and in 10.3% of people also taking a sulfonylurea [3]. The label tells prescribers to consider lowering insulin or sulfonylurea doses when Zepbound starts [3]. The Medication Guide asks you to tell your provider if you take either [6].

Signs of low blood sugar

Dizziness, sweating, confusion, headache, blurred vision, slurred speech, shakiness, fast heartbeat, anxiety, hunger, weakness or feeling jittery [6]. Treat it as your care team has taught you. Call 911 if someone passes out or cannot swallow safely.

Thyroid conditions

The boxed warning covers thyroid C-cell tumors in rats; it is unknown whether Zepbound causes them in people. Zepbound must not be used with a personal or family history of medullary thyroid carcinoma or MEN 2. The label says routine calcitonin tests or thyroid ultrasounds are of uncertain value for catching these tumors [3]. Report a neck lump, trouble swallowing, shortness of breath or lasting hoarseness [3].

Our sources do not address hypothyroidism, hyperthyroidism or thyroid hormone levels for Zepbound. They do flag thyroid pills indirectly: Zepbound slows stomach emptying, and the label advises monitoring oral drugs that need precise blood levels to work [3]. For comparison, the Wegovy label reports that its tablet raised levothyroxine exposure by 33% [7]; the Zepbound label in our research gives no such figure. Ask your prescriber whether your thyroid dose needs checking.

Pain relievers and other medicines

  • Ibuprofen and other pain relievers: not named in the Zepbound drug interaction section [3]. The label does warn that fluid loss from vomiting or diarrhea can cause acute kidney injury [3], so ask your pharmacist.
  • Birth control pills: switch to a non-oral method or add a barrier method for 4 weeks after starting Zepbound [3] and, per the tirzepatide labels, after each dose increase [2].
  • Other GLP-1 or tirzepatide drugs: not recommended with Zepbound [1].

Kidneys, liver, pregnancy and surgery

No Zepbound dose change is recommended for kidney or liver impairment [3]. Zepbound should be stopped once pregnancy is recognized [3]. Before any surgery or sedation, tell the team you take it, because aspiration has been reported under anesthesia [3]. The molecule-level page, tirzepatide interactions, covers blood pressure, breastfeeding and fertility in more depth.

Side effects in full are on Zepbound side effects. Coverage for diabetes vs weight is on Zepbound insurance and GLP-1 Medicare coverage.

Questions people ask

Is Zepbound used for diabetes?

Zepbound is approved for weight management and sleep apnea, not diabetes treatment [1]. The same drug is sold as Mounjaro for type 2 diabetes in adults and children 10 and older [2].

Is Zepbound approved for type 2 diabetes?

No. Its approved uses are weight reduction in adults with obesity or overweight with a weight-related condition, and sleep apnea in adults with obesity [1]. Adults with type 2 diabetes were included in its Study 2 weight trial [3].

Does Zepbound lower A1C?

In Study 2, adults with type 2 diabetes saw HbA1c fall 2.1 points on 10 mg and 15 mg vs 0.5 on placebo at 72 weeks [3].

Can Zepbound cause diabetes?

New-onset diabetes is not among the Zepbound label's warnings, and in Study 1 HbA1c fell 0.4 points in adults without diabetes [3].

Can Zepbound cause thyroid problems?

Zepbound caused thyroid C-cell tumors in rats; whether it does in people is unknown. It must not be used with a personal or family history of medullary thyroid carcinoma or MEN 2 [3].

Can you take Zepbound if you have hypothyroidism?

Hypothyroidism is not a contraindication in our sources. The label advises monitoring oral drugs that need precise blood levels, because Zepbound slows stomach emptying [3]. Ask your prescriber about your thyroid medicine.

Can type 1 diabetics take Zepbound?

Zepbound's label covers weight and sleep apnea, not diabetes treatment [1], and our sources hold no label statement on type 1 diabetes. Ask your endocrinologist.

Can you take ibuprofen with Zepbound?

Ibuprofen is not named in the Zepbound drug interaction section [3]. Check with your pharmacist, especially if you have vomiting or diarrhea, which the label links to kidney injury [3].

What medicine is Zepbound?

Zepbound is tirzepatide, a once-weekly injection by Eli Lilly that activates GIP and GLP-1 receptors [4] [3].

Can I take Zepbound with insulin?

The label addresses this pairing: it tells prescribers to consider lowering the insulin dose when Zepbound starts. In the diabetes trial, low blood sugar occurred in 4.2% on Zepbound vs 1.3% on placebo [3].
