GLP-1 diet plan: what to eat on semaglutide or tirzepatide

There is no official GLP-1 diet plan: the FDA labels for Zepbound and Foundayo say to use the drug with a reduced-calorie diet and increased physical activity, and neither lists specific foods or meals [1][2]. What the labels do set are timing rules, such as taking the Wegovy pill on an empty stomach with no more than 4 ounces of plain water and waiting at least 30 minutes before eating [3].

A GLP-1 diet plan is the way a person eats while taking a GLP-1 drug such as semaglutide or tirzepatide; the drug labels call for a reduced-calorie diet but do not define a specific plan.

We list only rules printed on the labels. Ozempic's food guidance was not part of our label review.

The Wegovy pill rule matters most for daily routine: breakfast, coffee and morning medicines all wait at least 30 minutes [3]. See Wegovy pill cost, Foundayo and our guides to using tirzepatide and using semaglutide.

Eating when nausea and other stomach effects hit

Stomach and gut effects are the most common problems. On Zepbound, nausea affected 25% to 29% of people across doses, diarrhea 19% to 23%, vomiting 8% to 13% and constipation 11% to 17%, vs 8%, 8%, 2% and 5% on placebo [5]. On Wegovy 2.4 mg, nausea affected 44% and constipation 24%, vs 16% and 11% on placebo [3]. In SURMOUNT-1, most stomach effects were mild to moderate and happened mainly while the dose was going up [6].

The labels do not give a food list for these effects. Tell your prescriber if you cannot keep food or fluids down: both labels warn of acute kidney injury from fluid loss and of severe stomach and gut reactions [5][3]. Zepbound is not recommended for people with severe gastroparesis [5]. More detail is in GLP-1 side effects.

Protein and keeping muscle

Eating less means the body loses some lean mass with fat. In a SURMOUNT-1 substudy, about 25% of the weight lost on tirzepatide was lean mass and 75% was fat [7]. No label sets a protein target. We could not confirm the protein numbers in a 2025 joint nutrition advisory from four US obesity and nutrition groups by Oct 17, 2026, so we do not print them here. Ask your prescriber or a registered dietitian for a target, and read how to keep muscle on a GLP-1.

Medicines, meals and blood sugar

  • Diabetes medicines. Low blood sugar is more likely with insulin or a sulfonylurea. In Zepbound's type 2 diabetes trial, 4.2% had low blood sugar below 54 mg/dL on tirzepatide vs 1.3% on placebo, and 10.3% of those also taking a sulfonylurea [5].
  • Birth control pills. Tirzepatide can make oral hormonal birth control less effective. The label says to switch to a non-oral method or add a barrier method for 4 weeks after starting and after each dose increase [4]. Foundayo's label says the same for 30 days [2].
  • Thyroid pills. With the Wegovy pill, levothyroxine exposure rose 33% in testing; the label advises monitoring oral drugs with a narrow safety range [3].

See tirzepatide interactions and semaglutide interactions.

Gallbladder, pancreas and surgery warnings

The Zepbound and Wegovy labels both warn of acute gallbladder disease and acute pancreatitis [5][3]. Severe stomach pain that will not go away needs a call to your prescriber right away. Both labels also warn that food or liquid can be breathed into the lungs during general anesthesia or deep sedation, so tell the care team you take the drug before any planned procedure [5][3].

After you stop the drug

In SURMOUNT-4, people switched from tirzepatide to placebo regained 14.0% of body weight from week 36 to 88, while those who stayed on it lost another 5.5% [8]. After stopping semaglutide in the STEP 1 extension, people regained 11.6 of 17.3 percentage points within a year [9]. No study in our sources tested a specific diet for keeping weight off after stopping. Brand guides: tirzepatide diet, semaglutide diet, Wegovy diet, Mounjaro diet, Ozempic diet.

When to call for help

Stop and call your prescriber for severe stomach pain that will not go away, which can be a sign of pancreatitis [5][3]. If you take insulin or a sulfonylurea, watch for low blood sugar [5]. These drugs carry a boxed warning for thyroid C-cell tumors seen in rodents; do not use them with a personal or family history of medullary thyroid carcinoma or MEN 2 [5][3]. Stop Zepbound or Wegovy when pregnancy is recognized, and stop semaglutide at least 2 months before a planned pregnancy [5][3]. For signs of a serious allergic reaction, call 911.

Questions people ask

Is there an official GLP-1 diet plan?

No. The Zepbound and Foundayo labels say to use the drug with a reduced-calorie diet and increased physical activity, and neither names specific foods or meals [1][2].

What should I eat on semaglutide?

The Wegovy label does not list foods. It does set rules for the Wegovy pill: take it on an empty stomach in the morning with up to 4 oz of plain water and wait at least 30 minutes before eating, drinking anything else or taking other oral medicines [3].

Can I take Zepbound or Mounjaro with food?

Yes. The tirzepatide labels say the weekly shot can be given any time of day, with or without meals [4].

How much protein do I need on a GLP-1?

No GLP-1 label sets a protein target. We could not confirm the protein numbers in the 2025 joint nutrition advisory as of Oct 17, 2026. In a SURMOUNT-1 substudy, about 25% of weight lost on tirzepatide was lean mass [7], so ask your prescriber or a registered dietitian for a target.

What should I eat when GLP-1 nausea hits?

The labels give no food list for nausea. Nausea affected 25% to 29% of people on Zepbound vs 8% on placebo [5]. Call your prescriber if you cannot keep fluids down, because the labels warn of kidney injury from fluid loss [5].

Does a GLP-1 change blood sugar if I have diabetes?

Low blood sugar is more likely with insulin or a sulfonylurea. In Zepbound's type 2 diabetes trial, 4.2% had low blood sugar below 54 mg/dL vs 1.3% on placebo, rising to 10.3% with a sulfonylurea [5].

What happens to my weight if I stop the drug?

In SURMOUNT-4, people switched to placebo after 36 weeks on tirzepatide regained 14.0% of body weight by week 88 [8]. After stopping semaglutide in the STEP 1 extension, people regained 11.6 of 17.3 percentage points within a year [9].
